脓毒症微循环功能障碍的评估及其临床意义 被引量:6

摘要 脓毒症的病理生理过程涉及机体一高度复杂且整合的应答反应,包括各种细胞、炎症介质及凝血系统的激活.1994年Lam C应用活体显微镜对血压稳定的脓毒症大鼠微循环进行在体观察,首次提出脓毒症早期存在微循环变化;1999年正交偏振光谱成像技术(OPS)进入临床应用,证实舌下微循环与内脏器官微循环变化基本一致,使得国内外学者对脓毒症早期存在微循环变化达成共识.近年来越来越多的研究提示,微循环功能障碍是全身炎症反应综合征(SIRS)、脓毒症及其所致多脏器功能障碍综合征(MODS)发生发展中重要的病理生理基础[1-2],早期改善微循环的治疗是保护重要脏器功能的根本措施.本文就脓毒症微循环障碍的发生机制、评估方法及其临床意义做一综述.
