骨质疏松症松质骨连接性参数改变与计算机辅助测量 被引量:11

摘要 目的 松质骨连接性参数主要指小梁骨的节点数和游离末端数 ,是反映松质骨显微构筑的重要参数 ,而松质骨的构筑情况可直接影响其力学性能。本研究旨在观察骨质疏松症小梁骨连接性参数的改变 ,并介绍一种计算机辅助测量方法。方法 对老年人和青年人股骨头负重区与非负重区、卵巢切除后 7周大鼠椎体骨以及采用重组人生长激素治疗后的椎体标本 ,进行节点数和游离末端数的测量。并发展了小梁骨节点数和游离末端数的计算机图像自动分析系统。即采用 5 μm硬组织切片将其光镜图像输入计算机 ,用我们自行编制的程序进行分析 ,结果与人工计量方法比较 ,具有很好的相关性 (分别为 r=0 .95 ,r=0 .99)。结果 与青年人相比 ,老年人股骨头标本的节点数明显下降 ,游离末端数明显上升(均为 P<0 .0 5 ) ,卵巢切除 3个月后椎体小梁骨的节点数和游离末端数也呈现同样的变化趋势。而重组人生长激素治疗后这些参数有一定的改善 ,其中骨小梁节点数和末治疗组比较 ,其增加有统计学意义( P<0 .0 5 )。结论 骨小梁节点数的下降和游离末端的增加是骨质疏松症的主要病理特征 ,这两个参数的计测对骨质疏松症的诊断、骨折风险预测和药物疗效评定具有较大的参考价值。 Aim:To xplore the changes of cancellous bone connectivity of osteoporosis and develop a computer- aided measurement method.Methods:we measured the numbers of nodes and free ends in the weight- bearing and non- weight- bearing areas of femoral head in the aged and the youth,in the vertebrae of rats 7weeks after ovariectomy and in the speciments of vertebrae after treatment with recombinant human growth hormone.Also,a computer- aided image analysis system for the numbers of the nodes and free ends of trabeculae was developed,for examining 5μm undecalcified sections with the program drawn up by ourselves,the results being well correlated with those of manual measurements( r=0 .95 ,r =0 .99respectively) .Results:It was found that the number of nodes in the speciments of femoral head of the aged markedly reduced as compared with thatin the youth,while the number offree ends marked- ly increased( P<0 .0 5 in both cases) ;the two parameters of vertebral trabeculae 3months after ovariec- tomy showed the same tendency of changes.Some improvements were observed in the specimens treated with recombinant human growth hormone,as compared with the untreated group,the increase in the number of trabecular nodes was statistically significant( P<0 .0 5 ) .Conclusion:The results suggest that the decrease in the number of trabecular nodes and increase in the number of free ends are the principal pathological characteristics of osteoporosis.The measurement of these two parameters is of great value as reference for the diagnosis of osteoporosis,the prediction of the risk of fracture and the e- valuation of pharmaceutical effects
