后入路椎体间融合笼治疗退行性下腰椎不稳症 被引量:2

Using Plif-Cage Technique for Treatment of Degenerative Instability of Lower Lumbar Spine

摘要 目的 :为了达到不稳定椎体间早期可靠的植骨融合效果。从 1996年起 ,我院采用后入路腰椎体间融合笼(PLIF Cage)技术治疗 2 0例退变性下腰椎不稳定症 ,取得满意疗效。方法 :2 0例退变性下腰椎不稳症 ;男 5例 ,女 15例 ;其中L4~ 5不稳定 12例 ,L5S1不稳 8例 ;采用单Cage植入 13例 ,双cage植入 7例 ,术后卧床 3周带腰围下地活动。结果 :除 1例因为术中选用Cage不当导致术后松动需二次手术外 ,余术后Cage植入效果良好 ,3个月后均见椎节骨性融合。结论 :PLIF Cage技术对早期椎节的稳定和后期的椎节骨性融合均具有良好的疗效 ,但应用时要注意术前定位、术中神经保护及Cage选择。 Objective: Performing the posterior lumbar interbody fusion (PLIF) can relieve the compression of the cauda nerve and stabilize the lumbar spine at the same time. In order to get an early effective implantive results and improve effect of the interbody osteosis fusion in latter period, our hospital had used Plif Cage Technique to treat twenty cases of degenerative instability in the lower lumba spine since 1996. We have achieveod great healing effect. Method: There were twenty cases of degenerative lower lumbar instability; five male and fifteen female; twelve cases insability occured in L 4~5, eight cases in L5-S1. In thirteen cases, we used single cage implant unit and double cages implant units for other eight cases, postoperatively, patients were allowed out of bed with girdle after three weeks.Result: All cage implants were fixed well, except one case which used a unfit unit in operation needing reoperation. All cases could be obsevered the osteosis union in the interbody space from X-ray film after three months. Conclusion: Plif Cage technique is an advanced and scientific method for treatment of lower lumbar degenerative instability. It not only has good stability in interbody fusion of early period but also improves interbody osteosis fusion in latter period. It is easy to be mastered and deserves popularigation, but we must pay attention to make orientation before operation, avoid nerve injury and choose the fit cage units in operation.
