摘要
目的 探讨超声诊断微小肾结石的可靠性。方法 在离体猪肾微小肾结石模型建立的基础上,应用7.5MHz探头,超声诊断不同大小(分别为1~3mm和3~5mm)的肾结石。结果 超声对诊断3~5mm结石具有较高的敏感性和阳性预测值,在临床诊断中具有实用价值。对1~3mm结石,超声诊断敏感性和阳性预测值均低,分别为41.4%和34.3%,且以假阳性诊断较多尤为显著,应用超声准确区分1~3mm结石与肾脏集合系统的强回声非常困难,不能满足临床诊断需要。结论 超声对3~5mm肾结石的诊断较为可靠,而对小于3mm肾结石诊断结果的可靠性极低,没有临床实用价值。术中超声应用时,超声测量结果在3.
Objective To evaluate the reliability of ultrasonic diagnosis of renal micro calculus.Methods Based on the establishment of renal micro calculus model in vitro,the reliability of ultrasonic diagnosis was evaluated. 7.5 MHz probe was adoped.According to the diameter,calculi were divided into two groups,1~3 mm and 3~5 mm in diameter.Results With relatively high sensitivity ( 83.3 %) and positive predict value ( 74.3 %),ultrasonic diagnosis of bigger stones (diameter between 3 mm to 5 mm) was reliable.On contrast,they were lower in the other group.Sensitivity and positive predict value were 41.4 % and 34.3 %,respectively.It was difficult to differ the hyperecho of stones with diameter 1~3 mm from that of the renal collecting system including renal pelvis,renal sinus and vessels by ultrasonography.Conclusions For stones of 3~5 mm,the ultrasonography was a useful and valuable aid of diagnosis,but it was no value in dealing with stones of 1~3 mm.The diagnosis of renal micro calculus could be made only when a hyperecho bigger than 3.5 mm was seen.
