摘要
内镜黏膜下剥离术(endoscopic submucosal dissection,ESD)在消化道肿瘤的治疗中发挥着重要作用,应用ESD能一次性完整切除较大的病变组织,提供完整的病理诊断资料,术后随访也证明了其优越性。目前国内应用ESD治疗消化道肿瘤的临床报告呈逐年递增趋势。ESD的手术操作分为标记、抬起、切缘、剥离和创面处理等五步,出血和穿孔是治疗中最常见的并发症。本文就ESD应用于消化道肿瘤治疗的适应证和禁忌证、手术步骤和注意事项、并发症及其处理、技术评价等内容作一综述。
Endoscopic submucosal dissection （ESD） plays an important role in the treatment of gastrointestinal tumors. It is possible to completely resect submucosal lesions and provide sufficient pathological information,postoperative follow-up have proved its superiority. Now it has been widely used for the treatment of gastrointestinal tumor. ESD operation consists of marking,injection to elevate the lesion, pre-cutting, dissecting and wound processing. Bleeding and perforation were the most frequent complications. This review describes current advances in the treatment of gastrointestinal tumors by ESD.
