摘要
目的:研究舌侧活动翼矫治技术治疗均角型双颌前突的临床疗效。方法:选取均角型双颌前突患者31例(29°<SN/MP<40°),根据患者对矫治器的主观要求分为舌侧活动翼矫治器组和唇侧活动翼矫治器组。舌侧治疗组11例,唇侧治疗组20例。所有病例均拔除4颗第一前磨牙进行矫治。将两组治疗前后的X线头影测量数据和两组治疗前后测量数据的差值进行统计学分析。结果:矢状方向:①舌侧组上下切牙分别内收4.89mm和3.68mm,直立16.91°和10.61°。且两组差值的比较无统计学意义。②矫治后两组磨牙均有所前移,唇侧组上磨牙前移2.31mm,下磨牙前移2.72mm。舌侧组上磨牙前移2.5mm,下磨牙前移2.72mm,并且两组差值无明显差异,均达到中强支抗。垂直方向:①舌侧组上磨牙伸长1.03mm,下颌磨牙伸长0.99mm。②两组的Y轴角、面角、FMA、SN/MP治疗前后均没有统计学意义,OP/FH有所增加,后面高有所增加。软组织方面:①两组的上下唇厚度明显减少、上下唇明显内收、鼻唇角增大。②舌侧组上切牙和上唇内收量的比为1:0.4009,下切牙和下唇内收量的比为1:0.7092。结论:舌侧活动翼能一定程度上弥补舌体解剖上的个体差异性。在支抗维护与垂直方向控制上效果较好,上下颌前牙突度和倾斜度明显减小,软组织侧貌有明显改善,能有效地改善双颌前突畸形。
Objective To study the clinical effect of treating the normal group bimaxillary protrusion with lingual activewing appliance. Methods 31 patients with the normal group bimaxillary protrusion were divided into two groups by subjective requirements of the patient to the appliance,Their SH/MP angle demand between 29°to 40°.two groups were the lingual active- wing and the lip active-wing.All patients were pulling out four of the first premolar for correction. Cephalometric analysis were carried out before and after treatment, the results and the difference of before and after treatment of two groups were analyzed with statistics. Results Sagittal direction: (1)the adduction of centre incisor on the maxillary and mandible were 4.89mm 和 3.68mm,and the erection respectively were 16.91°和10.61°.The difference did not significantly change in two groups.(2)all cases had moved after correction, of maxillary and mandible molar were 2.74mm and 2.37mm of the lingual active-wing.The difference did not significantly change in two groups, there belonged to the moderate anchorage.Vertical direction:(2) Elongation of maxillary and mandible molar were 1.03 mm and 0.99 mm of the lingual active-wing. (2)Y axis,facial angle,FMA,PP/MP,SN/MP did not significantly change than before treatment of all cases. OP/FH and Co-Go had increased.Soft tissue:the thickness of upper and lower lips decreased significantly,the protrusion of upper lip and lower lip decreased significantly.Nasal angle of the lip had increased.Cetral incisor of the maxillary and upper lip retreated of the ratio for 1:0.357,and Cetral incisor of the mandible and lower lip backed of the ratio for 1:0.8799 in the lingual active-wing. Conclusion Lingual active-wing can make up a certain degree wing tongue anatomical differences.The anchoragse and vertical direction were controlled preferably.The inclination and protrusion of the upper and lower anterior teeth decreased significantly.Facial profiles were improved significantly after orthodontic treatment in high angle.Clinical curative effect is good.
