扭动模式超声乳化白内障吸除的临床应用 被引量:4

Application of OzilTM torsional technology in phacoemulsification for extraction of a catarect

摘要 目的探讨扭动模式超声乳化用于白内障吸除术的效果。方法单纯老年性白内障患者132例(184眼),随机分为两组,分别采用扭动模式(A组,92眼)和常规模式(B组,92眼)施行白内障吸除术。术前对核硬度分级。记录术中所用的超声时间及超声能量。并比较术后角膜透明程度及最佳矫正视力。结果两组均采用负压400mm Hg,流量40ml/min。A组超声能量为0-10%;B组超声能量为30%-40%,爆破模式。在Ⅰ、Ⅱ级核患者,A组术中所用的超声能量及超声时间分别为(7.4876±4.1235)%和(21.2722±16.0634)s,均明显少于B组的(10.3415±3.6663)%和(33.1488±20.3963)s(P<0.01);在Ⅲ、Ⅳ级核患者,A组术中的超声能量及超声时间分别为(12.1627±5.0797)%和(34.3961±19.7183)s,均明显少于B组的(14.8157±6.1555)%和(70.0784±37.0080)s(P<0.01)。与B组比较,A组Ⅲ、Ⅳ级核患者术后1、3d的角膜水肿较轻、最佳矫正视力更好(P<0.05)。结论扭动模式超声乳化较之常规超声乳化更为高效和安全,术后早期易获得理想视力。 Objective To investigate the outcomes of OzilTM torsional technology in phacoemulsification for extraction of a cataract. Methods A total of 132 patients with cataract(184 eyes) was divided into two groups of A(92 eyes,using OzilTM torsional technology) and B(92 eyes,using conventional technology).The nuclei were graded according to the hardness.The operative outcomes were evaluated with the equivalent phaco time(EPT),100% torsional power in group A or the phaco power in group B,cornea clarity,and best corrected visual acuity after surgery. Results The burst mode with the vacuum of 400 mm Hg was used during phacoemulsification.The power was 0-10% in group A,and 30%-40% in group B.For the cases with nuclei grade Ⅰ-Ⅱ,EPT and torsional power in group A were(7.4876±4.1235)% and(21.2722±16.0634) s,which were less than(10.3415±3.6663)% and(33.1488±20.3963) s in group B(P0.01).For the cases with nuclei grade Ⅲ-Ⅳ,EPT and torsional power in group A were(12.1627±5.0797)% and(34.3961±19.7183) s,which were less than(14.8157±6.1555)% and(70.0784±37.0080) s in group B(P0.01).Compared to group B with grade Ⅲ-Ⅳ nuclei,the cornea clarity and best corrected visual acuity were significantly better in group A on the 1st and 3rd day after operation(P0.05). ConclusionThe OzilTM torsional technology in phacoemulsification is better than conventional technology in reducing EPT actual power and in phacoemulsification in the early operative days,which improves the safety and efficiency for the extraction of a cataract.
