摘要
目的探讨几种自体输血方法在特发性脊柱侧弯矫形围手术期的作用和临床意义。方法将特发性脊柱侧弯矫形患者74例按使用自体血回输方法不同分成4组,分别为未用自体血组(A组)17例;单纯使用术前预存自体血组(B组)19例;单纯使用术中血液回输组(C组)20例;术前预存自体血联合术中血液回输组(D组)18例。比较各组患者的术中出血量和围手术期输异体血比例及输异体血量。结果各组术中出血量差异无统计学意义(P>0.05)。A组100%(17/17)需输异体血,平均847ml;B组63.5%(12/19)需输异体血,平均435mL;C组90%(18/20)需输异体血,平均528mL;D组11.1%(2/18)需输异体血,平均250mL。平均输血量A组明显多于B、C、D 3组(P<0.05,P<0.01),C组多于D组(P<0.01);输血例数与百分比A组高于B、D 2组,C组高于D组(P<0.01)。结论联合使用术前预存自体血、术中血液回输在特发性脊柱侧弯矫形围手术期应用能较大限度减少异体血液的输入;综合各种因素分析在特发性脊柱侧弯矫形围手术期,术前预存自体血技术最为经济,同时又可减少患者输入异体血的机会。
Objective To explore the role and clinical significance of several autologous transfusion methods in perioperative period of idiopathic scoliosis surgery. Methods Seventy - four cases of idiopathic scoliosis patients were divided into 4 groups by the use of autologous blood transfusion methods. Group A had not used autologous blood with 17 cases, group B simply used preoperative autologous blood with 19 cases, group C simply used intraoperative autotransfusion with 20 cases, and group D used preoperative autologous blood combined with intraoperative autotransfusion with 18 cases. The intraoperative blood loss, intraoperative autologous blood recovery, percentage of perioperative allogeneic blood transfusion, and allogeneic blood transfusion volume were analyzed among the 4 groups. Results There was no significant difference of intraoperative blood loss in each group. In group A, 100% of the patients had aUogeneic blood transfusion with .an average volume of 847mL. In group B ,63.5% of the patients received allogeneic blood transfusion with an average volume of 435mL. In group C ,90% of the patients received allogeneic blood transfusion with an average volume of 528mL. In group D, 11.1% of the patients received allogeneic blood transfusion with an average volume of 250mL. Conclusion Combined using preoperative storage, autologous washed blood reinfusion interoperation, allogeneic blood transfusion can be reduced to a great degree in perioperative period of idiopathic scoliosis surgery. Ofcourse, the method of preoperative storage was the most economical and effective method to reduce allogeneic blood transfusion.
