普外科住院患者营养状况评价及预后分析 被引量:3

PREVALENCE OF MALNUTRITION IN GENERAL SURGICAL PATIENTS:EVALUATION OF NUTRITIONAL STATUS AND PROGNOSIS

摘要 目的探讨普外科住院患者营养状况,并分析其与预后之间的关系,为今后的干预提供依据。方法采用改良主观全面评价法评判442例普外科非急诊住院患者营养状况,考察营养状况在不同年龄、不同疾病间的分布情况,并分析营养状况与病死率、并发症和住院时间之间的关系。结果普外科住院患者中存在不同程度的营养不良状况,总体营养不良的发生率为32.35%;≥60岁的老年患者营养不良发生率(48.23%)高于<60岁的发生率(24.92%);消化道疾病患者的营养不良发生率(36.79%)高于消化道外疾病患者(20.97%);患恶性疾病的营养不良发生率(55.93%)高于患良性疾病的发生率(20.39%)。营养不良患者的术后并发症发生率、病死率和住院时间均高于营养状况良好者。差异均具有统计学意义(P<0.01)。结论在临床工作中应强化对患者实施准确地营养评估,并据此给予合理的营养支持治疗,从而降低术后并发症的发生,提高患者的治疗效果和生活质量。 Objective To investigate the nutritional status of hospitalized patients who finished general surgery, and analyze the relationship between nutritional status and prognosis. Methods The nutritional status of 442 cases which were non -emergency general surgery hospitalized patients was evaluated using modified subjective comprehensive evaluation method, the distribution of nutrition at different ages and different diseases was studied to analyze the relationship of nutritional status, mortality, complications and length of hospital stay. Results There were varying degrees of malnutrition among hospitalized patients in general surgery, the overall malnutrition rate was 32.35% ;the incidence of malnutrition in elderly patients who were I〉60 years old (48.23%) were much higher than 〈 60 years old(24.92% ); the incidence of malnutrition in patients with gastrointestinal disease (36.79%) were higher than patients with other diseases (20. 97% ); the incidence of malnutrition in patients with malignant diseases (55.93%) were higher than patients with benign disease (20.39%). postoperative complication incidence, mortality and length of hospital stay of malnutrition patients were higher than those in good nutrition. These differences were statistically significant (P 〈 0. 01 ). Conclusion Assessed Accurate nutritional assessment for patients should be stressed in clinical work and reasonable nutrition therapy should be given to reduce the incidence of postoperative complications and improve patients, outcomes and quality of life.
