摘要
目的总结膀胱癌患者行膀胱全切输尿管腹壁造口术后的护理体会。方法回顾性分析本院2009年1月～2010年12月膀胱癌行膀胱全切输尿管腹壁造口术后43例患者的临床资料,并总结术后护理要点。结果 43例患者术程顺利,手术时间4～5h,中位时间4.5h;术中出血量300～900mL,中位出血量400.0mL;住院时间10～19d,中位时间12.4d。术后发生造口周围尿液性皮炎2例,输尿管造口狭窄1例;1例患者死亡,其余均痊愈出院。结论膀胱癌行膀胱全切输尿管腹壁造口术后患者需要长期佩带造口袋生活,术后会出现心理问题。护士应重视对患者的心理护理,同时保持输尿管支架管通畅,做好健康教育,预防术后并发症,以保证患者的手术取得成功,提高术后的生活质量。
Objective To sum up the experience in nursing patients with bladder cancer treated with the radical cysteetomy and ureteroeutaneostomy. Methods The clinical data of 43 patients with bladder cancer treated with the radical cysteetomy and ureterocutaneostomy from January 2009 to December 2010 in our hospital were retrospectively analyzed to sum up the key points of postoperative nursing care. Results All the operations were performed successfully. The operation duration was 4 - 5 h, median time 4.5 h. The volume of blood loss was 300 - 900 mL, median 400 mL. The hospitalization time was 10 - 19 days, median time 12,4 days. Two cases had urinary dermatitis around stoma and one case had ureteral stomatie stegnosis. All were cured and discharged except one case of death. Conclusions The bladder cancer patients are susceptible to mental issues in that they have to live with ostomy bags for a long time after receiving the radical cystectomy and ureteroeutaneostomy. Nurses should conduct mental care to them, keep the ureteral stents tube unobstructed and perform health education actively to prevent postoperative complications so as to ensure the success of the operation and improve the postoperative quality of life.
