摘要
目的探讨帕金森病(Parkinson's disease,PD)患者生活质量影响因素,为临床干预提供依据。方法采用问卷调查法,应用帕金森病生活质量量表(the 39-item Parkinson's disease questionnaire,PDQ-39)对 82 例 PD 患者生活质量情况进行调查,同时选用 H&Y(hoehn-yahr stage,H&Y)分级、帕金森病统一评分量表第 2 部分(unified Parkinson's disease rating scoreⅡ,UPDRSⅡ)和第 3 部分(unifiedParkinson's disease rating score Ⅲ,UPDRSⅢ)、汉密顿抑郁量表(hamilton depression scale,HAMD)、帕金森病睡眠量表(Parkinson's diseasesleep scale,PDSS)、蒙特利尔认知评估量表(montreal cognition assess,MOCA)分别评估 PD 患者疾病严重程度、日常生活能力和运动症状、抑郁水平、睡眠状况、认知功能等指标。了解 PD 患者生活质量影响因素。结果患者生活质量评分为(43.0 ± 14.7)分。影响 PD 患者生活质量主要因素按影响作用大小依次为 HAMD、UPDRSⅡ和 PDSS 评分(均 P < 0.05),3 者对 PDQ-39 的影响起决定作用的 57.7%。结论 抑郁、日常生活能力和睡眠障碍是影响 PD 患者生活质量的主要因素。临床上应重视对患者抑郁情绪、睡眠障碍等非运动症状的护理,同时完善生活辅助用具,提高患者日常生活能力,以便改善 PD 患者的生活质量。
Objective To investigate the factors influencing Parkinsonian quality of life (QOL). Methods 82 Parkinsonians involved in the investigation on quality of life by the 39-item Parkinson's disease questionnaire (PDQ-39). Meanwhile, the severity of disease, activities of daily living (ADL), motor function, depression, sleep state and cognitive ability were measured by Hoehn-Yahr stage (H&Y), Unified Parkinson's disease rating score (UPDRS II and III, Hamilton depression scale (HAMD), Parkinson's disease sleep scale (PDSS) and Montreal cognition assess (MOCA), respectively. Stepwise multiple linear regression analysis was carried out to test the independent effects of the selected variables on QOL in PD Patients. Result The greatest influencing factors on Parkinsonian QOL were HAMD, UPDRS ]] and PDSS (P 〈 0.05), together explaining 57.7% of influencing factors by PDQ-39. Conclusions The main factors influencing Parkinsonian QOI~ are depression, ADL and sleep disorders. Therefore, it is critical to improve depression and sleep disorders during the treatment of Parl^inson's disease. On the other hand, the life assistance facilities can be bettered for the improvement of QOL.
