摘要
为总结以急腹症就诊的结肠癌患者的外科诊治体会，回顾分析2000年1月至2011年6月我院收治的以急腹症就诊的27例结肠癌患者的临床资料。结果显示，27例患者术前诊断为急性阑尾炎6例，肠穿孔7例，肠梗阻8例，腹腔内出血3例，病因不明3例。全部行手术治疗，16例行右半结肠切除术，2例行全结肠切除术，5例行左半结肠切除术，1例行乙状结肠造口术，1例行右半结肠切除和回肠造口术，2例行经腹前切除术。平均手术时间为（148．87±47．32）min；平均术后住院（17．87±5．48）d。术后并发症：肺部感染4例，创口感染2例，肠瘘1例，短期腹泻2例。无围手术期死亡病例。结果表明，结肠癌急腹症不同于一般急腹症，术式选择以挽救生命为第一原则，依病情和病变部位而定，实施个体化方案，以达到延长生命之目的。
In order to sum the experience on diagnosis and treatment of the patients consulting a doctor due to their acute abdomen in colonic carcinoma surgery, author retrospectively analysed the clinical data from the 27 cases who were admitted in author＇s hospital from Jan. 2000 yr to ]un. 2011 yr. As resuhs,among the 27 cases 6 were diagnosed as acute appendicitis,7 as perforation of intestine,8 as intestinal obstruction, 3 as intraabdominal cavity bleeding, 3 as unknown cause~ they had all been subjected to operative real- ment ： 16 cases, to right-semi-colon resection 2 to whole colon resection, 5 to left-semi-colon resection, 1 to sigmoid-neostomy,1 to right-semi-colon resection and ileum-neostomy, 2 to anterior resection of rectum; average operative time was（148.87± 47.32） min, average postoperative hospital stay was（ 17.87±5.48） days;Postoperative complications were lung infection in 4 cases, wound infection in 2, intestinal leakage in one,short-term diarrhea in 2;no perioperative death case occured. Results show that acute abdomen of co- lonic carcinoma is different from common one,for which the first principle of procedure choice is saving life,according to the condition and lesion locus to perform individuality scheme so that attain the goal of prolonging life.
