沈氏装置稳定性的生物力学评价

摘要 目的 评价沈氏经椎弓根内固定装置的生物力学稳定性 .方法 采用 5具新鲜尸体脊柱 (T8- L5 )标本 ,制造 L1损伤模型 ,测量并比较标本在完整状态、损伤状态、沈氏装置固定、沈氏装置疲劳及 CD装置固定下的 T1 2和 L2之间的三维运动范围 ,并将沈氏装置与 CD装置固定下的运动范围进行比较 ,经统计学处理 ,评价沈氏装置的稳定性 .结果 与完整状态比较 ,损伤状态下脊柱标本在屈 /伸、左 /右侧弯以及左/右轴向旋转方向上的运动范围都显著增大 .沈氏装置固定后标本在屈 /伸、左 /右侧弯以及右轴向旋转方向上的运动范围小于完整状态或与完整状态无差异 ,在左轴向旋转时运动范围大于完整状态 .沈氏装置疲劳实验后在屈 /伸、左 /右侧弯方向上的运动范围与完整状态无差异 ,在左 /右轴向旋转方向上的运动范围显著大于完整状态 .沈氏装置与 CD装置相比较 ,在各运动方向上运动范围无差异 .结论  1沈氏装置在屈 /伸、左 /右侧弯方面具有良好的即刻稳定性和疲劳稳定性 ,达到或超过完整结构状态 ,在左 /右轴向旋转方面 ,即刻稳定性部分达到完整状态 ,疲劳稳定性未达到完整状态 . 2沈氏装置的稳定性在各运动方向上与 CD装置无差异 . AIM To evaluate the biomechanical stability of Shen's instrumentation in the management of thoracolumbar fractures. METHODS The L1 level injury was made in five fresh adult cadaver spine specimens (T8~L5). The ranges of movement (ROM) of T12~L2 in three dimensions were measured sequentially in the state of normal, injured, Shen's fixation, fatigued Shen's fixation and CD fixation (transpedicle device). The ROM was compared in each test group and control group, and in Shen's fixation group and CD fixation group. Student T test was used in statistical analysis. RESULTS The present results showed that the ROM of injured spine was increased statistically in all three dimensions. After the application of Shen's fixation, the ROM was decreased and there was no difference between Shen's fixation and the normal control statistically in flexion/extension, as well as left/right lateral bending and right axial rotation, but the ROM was larger in left axial rotation. The ROM of fatigued Shen's fixation group was close to that of normal group in flexion/extension, as well as left/right lateral bending movement, but larger than normal group in left/right rotation movement. There was no statistically difference between Shen's and CD fixation in ROM in three dimensions. CONCLUSION ①The stability of Shen's fixation is good in flexion/extension, and left/right lateral bending. In left /right axial rotation, it to some extent reaches the level of normal specimen. ② The stability of Shen's fixation is very close to that of CD fixation in the management of thoracolumbar fracture.
