20例胆管细胞型肝癌临床分析

摘要 ①目的评价胆管细胞型肝癌的临床特点和治疗方法以指导临床诊治,提高患者生存率。②方法回顾性分析20例胆管细胞型肝癌患者的临床资料。③结果胆管细胞型肝癌缺乏特异性临床表现,40%为意外发现;B超和CT对胆管细胞型肝癌的诊断率极低,仅5%,且有20%误诊为肝血管瘤;有75.0%的患者CA-199升高;根治性切除组1、3、5年生存率分别为81.8%、45.5%和36.4%;姑息性切除(R1切除)6例,1、3年生存率分别为16.7%和0。④结论胆管细胞型肝癌易被误诊为肝血管瘤;CA199和GGT、AKP为较好的胆管细胞型肝癌预示指标;胆管细胞型肝癌以手术治疗为主,根治性手术切除是获得长期生存的唯一途径。
