摘要
目的:研究计算机辅助设计和快速成型技术在单侧上颌骨缺损阻塞器修复中的应用价值。方法:4名患者均为肿瘤切除术后的单侧上颌骨缺损病例。于术后3个月对缺损区进行CT扫描,扫描图象利用Simplant-CMF软件来进行颌骨缺损的容积三维重建和分段式阻塞器设计;再通过快速成型技术加工上颌骨缺损模型;在获得的树脂模型上,利用灌注法和涂层法制作硅橡胶阻塞器。临床试戴分段式阻塞器,检查其固位情况及与口鼻腔黏膜的密合度;通过含水、低头鼓漱的方法检测口鼻腔封闭性。结果:计算机辅助设计和快速成型技术制作的分段式阻塞器固位良好,与口鼻腔黏膜组织密贴。口鼻腔封闭性检查结果:3例无漏出,仅1例有少量漏出液。结论:计算机辅助设计和快速成型技术能提高上颌骨缺损的阻塞器的精密度和修复效果,具有较高的临床应用和推广价值。
Objective: To study the application value of computer aided design and rapid prototyping techniques in obturator restorations of maxillary defect. Methods: Four patients with unilateral maxillary defect were involved in the study. Three months after maxillectomy, each defect was checked by CT scan, and imagings were processed and designed by the Simplant-CMF software. The data were transferred to the rapid prototyping equipment for manufacture of the resin models of'matrix. Correspondingly, silicone rubber obturators were made by perfusion and coating methods. Clinically, all sectional obturators were tried--in and the marginal fitness and retention were examined. Sealing ability between oral and nasal cavity would be checked by rinsing the mouth. Results.. The marginal fitness and retention of sectional obturators were very good. Sealing ability check results showed that slight waterspillage from nasal cavity could be found in only one patient, and no waterspillage in the other three ones. Conclu- sion: Computer aided design and rapid prototyping techniques can effectivelly improve the precision and restorative effect of obturators in maxillary defect, and is valuable for clinical application and extension.
