摘要
目的检测尿毒症患者桡动脉和血清中的胎球蛋白A,探讨其和血管钙化的关系。方法选取2007年1月至2010年12月大连医科大学附属第一医院肾内科收治的尿毒症患者32例,动静脉内瘘成形术时取废弃的桡动脉。另取上肢外伤患者9例,手术时取废弃的上肢血管作为对照组。血管行von kossa染色,茜素红染色检测钙化,免疫组化检测胎球蛋白A、α-SMA、cbfα1。同时检测血清中胎球蛋白A、甲状旁腺素(iPTH)、C-反应蛋白(CRP)等。结果尿毒症组血清胎球蛋白A低于对照组[(204.03±75.03)mg/L对(386.03±37.70)mg/L,P<0.01]。血清胎球蛋白A随血管钙化程度的加重逐渐减低。血管的胎球蛋白A染色积分随血管钙化程度的加重逐渐升高。血管钙化值与血清胎球蛋白A呈负相关(茜素红染色r=-0.572,P=0.001;Von kossa染色r=-0.518,P=0.002)。结论尿毒症患者血清胎球蛋白A水平以及桡动脉上的胎球蛋白A表达与血管钙化有关。
Objective To determine fetuin A levels in radial artery and serum, and investigate their correlation with vascular calcification in patients with uremia. Methods We enrolled 32 patients with uremia from the department of nephrology, First Affiliated Hospital ofDalian Medical University between January 2007 and Decembbr 2010. Radial arteries discarded during arterial venous fistulaplasty were selected, and the radial arteries at discarded upper limbs in 9 patients who had up- per extremity trauma were chosen during operation and served as controls. Vessels were subjected to Von Kossa staining,and calcification of arteries was examined via alizarin red staining. Expression levels of fetuin A, smooth muscle aetin c~ ( c^- SMA) and core binding factor c^l (cbfcH) were detected using immunohistoehemistry assays. Also determined were serum fetuin A, immunoactive parathyroid hormone (iPTH) and C-reactive protein (CRP) levels. Results Serum fetuiu A level was remarkably lower in patients with uremia as compared with that in controls [ ( 204. 03 ± 75.03 ) mg/L vs ( 386. 03 ±37.70) mg/L, P = 0. 000 ]. In addition, serum fetuiu A level was constantly on the fall as vascular calcification advanced. By contrast, the gradual increase in fetuin A staining score was associated with advanced vascular calcification. Serum fetuin A exhibited a negative correlation with vascular calcification score (alizarin red staining r = -0. 572, P = 0. 001 ;Von Kossa staining r = -0. 518, P = 0. 002). Conclusion Both serum fetuin A level and its expression in radial artery are correlated with vascular calcification in patients with uremia.
