经皮腔内血管成形术联合银杏叶提取物治疗糖尿病下肢动脉闭塞症的临床研究 被引量:8

Clinical Research of Percutaneous Transluminal Angioplasty Combined with Ginkgo Biloba Extract in Patients with Diabetic Lower Extremity Artery Blockage

摘要 目的:探讨经皮腔内血管成形术联合银杏叶提取物治疗糖尿病下肢动脉闭塞症的临床疗效和应用价值。方法:对20例患者24条患肢行经皮腔内血管成形术,术中、术后联合银杏叶提取物治疗,观察术前、术后7d和术后3个月患者疼痛积分、行走距离分级积分、踝/臂指数及下肢神经传导速度变化情况。结果:24肢中获成功手术21肢(87.5%),术后12个月再狭窄3肢(14.28%);术后7d时疼痛积分、行走距离分级积分、踝/臂指数较术前明显改善(P<0.05);术后3个月时疼痛积分、行走距离分级积分、踝/臂指数、下肢神经传导速度均较术前明显改善;术后3个月疼痛积分、行走距离分级积分和下肢神经传导速度较术后7d时亦明显改善。结论:经皮腔内血管成形术联合银杏叶提取物治疗糖尿病下肢动脉闭塞症具有手术安全、成功率高、疗效确切的特点,值得推广。 Objective: To evaluate the efficacy and significance of percutaneous transluminal angioplasty (PTA) combined with ginkgo biloba extract (GBE) in patients with diabetic lower extremity artery blockage. Methods: PTA was adopted in 24 legs (20 patients) with diabetic lower extremity artery blockage. The combined treatment with GBE was used during operation and after operation. Pain scales, walk-distance scales, ankle brachial index and lower extremity nerve conduct velocity in patients were observed before operation, 7 days and 3 months after operation. Results: Twenty-one legs were operated successfully (87.5%), 3 legs were re-strictured (14.28%) 1 year after operation. Pain scales, walk-distance scales and ankle brachial index were improved obviously 7 days after operation compared with those of pre-operation (P 〈 0.05). Pain scales, walk-distance scales, ankle brachial index, motor nerve conduct velocity and sense nerve conduct velocity were also improved obviously after 3 months of operation compared with those of pre-operation. Furthermore the pain scales, walk-distance scales and lower limb nerve conduct velocity were improved obviously 3 months after operation than those of 7 days after surgery. Conclusion: The treatment of PTA with GBE was a safe,recommendable method with higher success rate and creditable efficacy in patients with diabetic lower extremity artery blockage.
