摘要
目的观察埃索美拉唑一周三联疗法治疗十二指肠溃疡的近期与远期疗效。方法将经内镜证实的十二指肠溃疡Hp阳性患者随机分为EAC组45例和OAC组35例,EAC组2次/d口服埃索美拉唑20mg、阿莫西林1000mg、克拉霉素500mg,疗程1周;OAC组用药除用奥美拉唑20mg代替埃索美拉唑外,其余与EAC组相同,用药1周后停用抗生素,继续单用奥美拉唑20mg,2次/d,治疗3周停药。用药期间观察症状缓解情况和不良反应,疗程结束后复查内镜观察溃疡愈合情况,检测Hp,对两组愈合患者追踪随访1年,观察溃疡复发情况。结果 EAC组症状缓解率、溃疡愈合率、Hp根除率、溃疡复发率分别为100%、93%、86.7%、7.1%,OAC组分别为100%、97%、88.6%、59%,两组对比差异无统计学意义(P>0.05),两组均无严重不良反应。结论埃索美拉唑一周三联疗法治疗十二指肠溃疡具有很高的Hp根除率和溃疡愈合率,缓解症状迅速,复发率低,患者依从性及安全性好,费用亦较传统治疗方案降低。
Objective To determine the short and long term effect of Esomeprazole One-week triple therapy in treatment of duodenal ulcer with Helieobaeter pylori （Hp） infection. Methods 80 eases d Hp posi- tive duodenal ulcer were randomly divided into two groups, the EAC group （45 ease） received Esomeprazole 20 rag, Amoxieillin 1000 rag, Clarithromycin 500 rag twice daily for 1 week, the OAC group received Omeprazole 20 rag, Amoxieillin 1000 nag, Clarithromyein 500 mg twice daily for I week, then continue to oral Esomeprazolesingle for 3 weeks, improvement of symptoms and adverse effects were observed during treatment course, the healing rate and the eradication rate of Hp were evaluated and compared after treatment course, endoscopy was performed to study the relapse rate after 1 year within the healing duodenal ulcer. Results The relief rate of symptoms, the healing rate and the eradication rate of Hp in EAC group were 100%, 93%, 86. 7%, and thosein OAC group were 100%, 97%, 88.6%, the adverse effects were mild in both groups, me relapse rate was 7. 1% in EAC group versus 5.9% in OAC group （P 〉0. 05） , there was no significant difference between the two groups. Conclusion The short and long term effect of esomeprazole based one-week triple therapy curing duodenal ulcer for was excellent, the cost is lower than traditional therapeutic regimen.
