Clinical Application of Robertshaw Double Lumen Tube Incubation for Mechanical Ventilation in 350 Patients

摘要 目的:观察Robertshaw双腔支气管导管(DLT)插管、机械通气的临床应用效果。方法:随机选择350例剖胸手术患者实施Robertshaw左DLT插管。其中男性264例、女性86例,年龄13~81岁,体质量39~95 kg。DLT插管后,机械通气根据患者体质量不同,选择潮气量7~11 mL/kg体质量,通气频率12~13次/min,吸呼比为1∶2。双肺通气后20 min及单肺通气后20 min行动脉血气分析。结果:350例患者双肺通气20 min后血气分析结果与健侧单肺通气20 min后血气分析结果均在正常范围。结论:350例Robertshaw DLT插管、机械通气临床效果满意,因血气正常是呼吸功能的最终标志。该观察还表明,DLT临床应用,监测氧合的最好方法是动脉血气分析,不可忽视。 Objective To study clinical application effect of Robertshaw doable lumen tube intubation (DLT) for mechanical ventilation. Three hundred and fifty patients with thoracotomy were selected randomly (male 264, female 86; age ranged from 13 to 81; weight ranged from 39 to 95 kg). After Robertshaw DLT, the mechanical ventilation was performed. The patients were treated with tidal volume of 7-1hnL/kg, respiratory frequency of 12-13 times / minute and inspiratory to expiratory" ratio of 1:2. Arterial blood gas was analyzed 20 min later after the two-lung ventilation and single-lung ventilation, respectively, Arterial blood gas analysis results of 20 rain later after the two-lung ventilation and 20 min later after the single-lung ventilation were all within the normal range. The clinical effect of DLT for mechanical ventilation is satisfied. The observation also shows that, the best surveillance indicator of DLT clinical application and oxygenation is arterial blood gas analysis, which can not be ignored.
