摘要
目的探讨C反应蛋白(CRP)在老年急性脑梗死患者并发全身炎症反应综合征(SIRS)和多器官功能障碍综合征(MODS)中的临床意义。方法 120例老年急性脑梗死患者根据病情分为单纯急性脑梗死组(SACI组,n=75),并发SIRS组(SIRS组,n=45),并发SIRS后致MODS组(MODS组,n=29)。同期从我院健康体检者中选取年龄、性别与患者相匹配的健康者40例作为对照组,应用乳胶凝集比浊法测定各组的血清CRP水平并进行比较。结果 120例老年急性脑梗死患者并发SIRS 45例,发生率为37.5%;并发MODS 29例,发生率为24.2%;并发SIRS患者致MODS的发生率为64.4%。4组的CRP阳性率及CRP水平间差异均有统计学意义(P<0.01);无论是CRP阳性率还是CRP水平均为MODS组>SIRS组>SACI组>对照组,各组间比较差异均有统计学意义(P<0.05)。结论老年脑梗死患者可以并发SIRS或MODS,血清CRP水平可以作为患者是否并发SIRS或MODS的一项指标,对于脑梗死的临床诊断和病情评估具有重要意义,可为临床及时有效地干预提供依据。
Objective To investigate the clinical significance of C-reactive protein(CRP) in identifying systemic inflammatory response syndrome(SIRS) and multiple organ dysfunction syndrome(MODS) induced by senile acute cerebral infarction.Methods A total of 120 patients with elderly acute cerebral infarction were categorized into simplex acute cerebral infarction group(SACI group,n=75),complicated systemic inflammatory response syndrome group(SIRS group,n=45),and a group of multiple organ dysfunction syndrome caused by complicated SIRS(MODS group,n=29).Forty healthy persons with matched age and sex were set as the control group.The serum CRP was determined by latex agglutination turbidmetric method and compared among the groups.Results The incidence of SIRS was 37.5%(n=45);29 patients had complication of MODS and the rate was 24.2%;and 64.4% of the patients with SIRS had complication of MODS.The positive rate of CRP and the level of CRP were significantly different among the four groups(P0.01;MODS group SIRS group SACI group control group),and the difference was significant between each group(P0.05).Conclusion Acute cerebral infarction in elderly patients are often accompanied with SIRS or MODS.Serum CRP level can be a useful indicator for the complicated SIRS or MODS,and thus provides useful information for the the clinical diagnosis and evaluation of cerebral infarction.
