实时三维超声心动图评价冠心病患者左室重构及其与冠状动脉病变程度的关系 被引量:7

摘要 目的应用实时三维超声心动图对冠心病患者左室重构进行评价,探讨左室重构与冠状动脉病变程度的关系。方法对59例冠心病患者及24例正常人根据Gensini积分分为轻度病变组(<25分)、中度病变组(25~49分)、重度病变组(≥50分)及正常对照组4组,分别应用实时三维超声心动图测量其左室舒张末期容积(LV-EDV)、左室舒张末期心外膜容积(LVEDVepi)、左室收缩末期容积(LVESV)、左室射血分数(LVEF)、左室心肌质量(LVM)、左室重构指数(LVRI)等各项左室重构指标,分析比较相关差异,并用左室重构各测量值与Gensini积分进行相关性分析。结果冠心病患者各病变组的LVEDV、LVESV、LVM均大于正常对照组(P<0.05或<0.01),LVEF、LVRI均小于正常对照组(P<0.01);随着冠状动脉病变程度的加重,LVEDV、LVESV进行性增大(P<0.05或<0.01),LVEF、LVRI进行性减小(P<0.01)。除LVM与Gensini积分无显著相关性外,LVEDV、LVESV、LVEF及LVRI与Gensini积分均有较好的相关性(P<0.01)。结论冠心病患者发生左室重构且重构程度随着冠状动脉病变程度的加重而加重,实时三维超声心动图能有效评价冠心病患者左室重构,LVRI可作为评价左室重构的新指标应用于临床。
