摘要
目的:系统评价中西医结合与单纯西医治疗结直肠癌(Colorectal cancer,CRC)的疗效和不良反应。方法:计算机检索中国生物医学文献数据库、中国期刊网、万方数据库、重庆维普和灰色文献,并配合手工检索所有文献全文,由两位评价者分别对资料进行质量评价,用Revman 5.1进行Meta分析,主要对实体瘤近期疗效、生活质量、不良反应进行分析。结果:共纳入9个随机对照试验,目前资料的Meta分析结果显示:临床治疗有效率:异质性检验P=0.99,表明研究数据具有同质性,故采用固定效应模式。合并效应量OR=1.48,95%CI 1.01～2.08,P<0.05(P=0.04),差异有统计学意义,表明中西医结合治疗大肠癌的近期疗效优于单纯西药;患者生存质量改善:OR=3.48,95%CI 2.17～5.58,P<0.01(P<0.00001),有显著差异,提示中西医结合治疗组患者生存质量显著优于单纯西药组;不良反应发生:①白细胞减少情况OR=0.31,95%CI 0.20～0.49,P<0.01(P<0.00001),有显著统计学意义,提示中西医结合治疗组出现血液学毒性显著低于单纯西药组;②神经毒性OR=0.49,95%CI 0.30～0.81,P<0.01(P=0.005),提示中西医结合治疗组出现神经毒性显著低于单纯西药组。结论:中西医结合治疗大肠癌的疗效优于单纯西药。
Objective ： To compare the effect of integration of Traditional and Western Medicine with western medicine therapy for colorectal cancer. Methods ：Relevant literatures were reviewed via database and assessed via randomized controlled trial （RCT） scale. All reviewed literatures were assessed via Jadad quality scale. Data statistical analysis were performed by RevManS. 1 software. Results ： Clinical efficient ： since test for heterogeneity ： P = 0.99, it indicates the data has homogeneity, so we use the fixed effect model. OR = 1.48,95 % CI 1.01 - 2.08, P 〈 0.05 （ P = 0.04 ）. It means that integration of Traditional and Western Medicine treatment for colorectal cancer in improving clinical symptom has obvious advantages compared with western medicine. For evaluation of quality of life, OR = 3.48,95% CI 2.17 - 5.58, P 〈 0.01 （P 〈 0. 00001 ）. It means that integration of Traditional and Western Medicine treatment for colorectal cancer in improving quality of life has obvious advantages compared with western medicine. For the main toxicity ： （1）occurrence of hematological toxicity： OR = 0.31,95 % CI 0.20 - 0.49, P 〈 0.01 （ P 〈 0. 00001 ） ; （2）occurrence of Neurotoxicity ： OR = 0.49, 95% CI 0.30-0.81 ,P 〈0.01 （P = 0. 005 ）. They mean that integration of Traditional and Western Medicine treatment for colorectal cancer in reducing toxicity has obvious advantages compared with western medicine. Conclusion： Integration treatment for colorectal cancer shows curative effect, and there are obvious advantages compared with western medicin.
