摘要
目的 探讨经小脑蚓部手术后口咽失用与缄默综合征的解剖和病理生理学基础。方法 回顾 37例后颅凹病变手术病人的病历资料 ,参照小脑的功能 ,并结合皮层切口的部位进行小脑分区 ,以便考察切口部位与本综合征的关系。结果 37例中 2例出现了口咽失用 -缄默综合征 ,切口部位均位于中下蚓部。结论 中 -下蚓部存在调控口、咽运用和言语的传导通路或结构 ,该部位切开后部分病人即可能出现口咽失用和缄默 。
Objective To probe the anatomical and pathophysiological bases of oral pharyngeal apraxia and mutism syndrome(OPAMS) following percerebellar vermis operation.Methods Basing on the retrospective analysis of 37 cases of operations of posterior cranial fossa,authors divided the cerebellum into several regions with respect to the cerebellar function and the site of incision so as to conform the relation between the site of incision and occurrence of OPAMS Results Two cases presented with OPAMS in 37 cases and the site of incision were located at middle inferior vermis.Conclusion Incision of middle inferior vermis may be resulted with OPAMS,especially in children.
