摘要
目的观察益生菌对结直肠癌患者术后免疫力、炎性反应以及手术预后的影响。方法 2008年3月—2010年12月行结直肠癌切除术的患者80例随机分为观察组和对照组。对照组患者术前5d始常规肠内营养,观察组在对照组基础上口服益生菌。2组患者术前1 d、术后1、9d进行免疫指标IgG、IgM、IgA以及炎性反应指标白细胞介素-6(interleukin-6,IL-6)、C反应蛋白(C-reactive protein,CRP)检测,并观察患者术后感染及其他不良反应的发生情况。结果术前2组患者免疫指标IgG、IgM、IgA以及炎性反应指标IL-6、CRP差异无统计学意义(P>0.05)。术后第1天2组患者上述免疫指标及炎性反应指标水平较术前降低(P<0.01),并且组间比较差异有统计学意义(P<0.01)。术后第9天2组患者免疫指标以及炎性反应指标水平较术后第1天升高(P<0.01),并且组间差异有统计学意义(P<0.01)。术后感染发生率观察组低于对照组(P<0.05),其他不良反应发生率差异无统计学意义(P>0.05)。结论术前在常规肠内营养基础上补充益生菌可提高机体免疫状况并降低炎性反应,减少术后并发症发生。
Objective To investigate the effect of preoperative use of probiotics on immunity, inflammation and prognosis of patients undergoing colorectal cancer resection. Methods Eighty patients, who underwent colorectal cancer resection from March 2008 to December 2010, were randomized into treatment group and control group. Patients in the control group received routine enteral nutrition alone from 5 days before operation and patients in the treatment group received oral probiotics with routine enteral nutrition. Parameters of immunity （ IgG, IgM and IgA ） and inflammation （ IL - 6, CRP） were detected. Infection and other adverse reactions were also observed. Results Parameters of immunity （IgG, IgM and IgA ） and inflammation （IL- 6, CRP ） were not significantly different in both groups before operation （P 〉 0.05 ）. Parameters of immunity and inflammation were significant decreased on 1 st day after operation （P 〈 0.01 ） compared ~o those before operation, and there were significant difference in both groups （P 〈 0.01 ）. Parameters of immunity and inflammation were significantly increased on 9th day after operation compared to those on 1st day after operation （P 〈0.01 ） ,and there were significant difference in both groups （ P 〈 0. 01 ）. Postoperative infectious complication in the treatment group was less than that in the control group （ P 〈 0. 05 ）, but other adverse reactions had no signifeant differece between the two groups （P 〉 0. 05 ）. Conclusion In patients with eoloreetal cancer, supplementation of probiotics before surgery can improve immunity and reduce inflammatory responds of surgery,and reducethe postoperative infectious complications.
