摘要
目的 检测转化生长因子 β(TGF β1,TGF β2 ,TGF β3)在增生性瘢痕和表浅性瘢痕中的表达 ,探讨其对增生性瘢痕的形成及α 平滑肌肌动蛋白 (α SMA)表达的可能作用。方法 采用免疫组化法检测 2 8例增生性瘢痕 ,19例表浅性瘢痕和 16例正常皮肤组织中TGF β和α SMA的表达水平 ,并按年龄 ,性别 ,病程分组进行比较及TGF β和α SMA相关性分析。 结果 ①TGF β和α SMA在正常皮肤组织中几乎无表达 ,而在增生性瘢痕和表浅性瘢痕中有不同程度的表达。②在增生性瘢痕中TGF β1,TGF β2 ,α SMA均有较强表达 ,与正常皮肤和表浅性瘢痕相比较差异有非常显著意义 (P <0 0 1)。③TGF β3在表浅性瘢痕和女性患者以及病程 1年以上的增生性瘢痕中表达较强。④少儿和成年组间TGF β和α SMA表达差异无显著意义。⑤TGF β和α SMA相关性分析显示TGF β1+TGF β2 -TGF β3的积分代数和与α SMA计数值呈正相关 ,r =0 .5 7,TGF β3与α SMA呈负相关 ,r =- 0 .80。结论 ①瘢痕的增生和挛缩是TGF β三种亚型的综合调节作用的结果 ,TGF β1和TGF β2 与α SMA的表达以及瘢痕的增生和挛缩有关 ,TGF β3与瘢痕增生的缓解有关。抑制TGF β1和TGF β2 或促进TGF β3的表达可能有益于防治瘢痕的增生和挛缩。
Objective To detect the expressions of TGF β(TGF β 1,TGF β 2,TGF β 3)and α SMA and to investigate the probable effects of TGF β in the formation of hypertrophic scars as well as the expression of α SMA in the scars. Methods 28 samples of hypertrophic scars,19 samples of flat scars and 16 samples of normal skin have been examined immunohistochemically for the expressions of TGF β and α SMA.Then a statistical and correlative analysis was carried out among the groups based on patient age,sex and scarring time.Results ①TGF β and α SMA have no expression in normal skin,but have varied expressions in hypertrophic scars and flat scars. ②TGF β\-1,TGF β\-2 and α SMA expressed higyly in hypertrophic scar in comparison with normal skin and flat scar with a significantly statistical difference (P<0.01) . ③There was a high expression of TGF β\-3 in the flat scar and some hypertrophic scar that were from the female group or the scar over one year.④There was no statistical difference between the groups of teenage and adults.⑤The correlative analysis of TGF β to α SMA demonstrated that the algebraic value of TGF β\-1+TGF β\-2-TGF β\-3 correlates positively to α SMA ( r =0.57).TGF β\-3 correlates negatively to α SMA(r=-0.80).Conclusions ①The formation and contraction of scar result from the synergic effects of TGF β\-1,TGF β\-2 and TGF β\-3.TGFβ 1,TGF β\-2 correlate the hyperplasia and contraction of scar,whereas TGF β\-3 correlates the maturing of scar.To suppress the expression of TGF β\-1,TGF β\-2 or promote the expression of TGF β\-3 would be beneficial to prevention of hyperplasia and contraction of scar.②It is reasonable to postulate that the female have stronger resistance to scar hyperplasia in that they have stronger expression of TGF β\-3,which deserves further clinical observation and ivestigation.③The age has no influence on the expression of TGF β and α SMA.
