摘要
总结了11例血小板减少症患者行心脏直视手术的护理。术前为了减少穿刺等治疗引起的出血,各项操作和检查均集中进行,并且操作轻柔,静脉输液用留置针。术后重点观察手术切口及引流管周围有无出血和渗血,发现出血和渗血并及时加压包扎止血;术后1～9h专人挤压胸腔引流管,保持引流管通畅,密切观察引流量和颜色的变化;在监测中心静脉压的同时快速输注血小板,观察输血反应及止血效果;严密监测心率、血压、呼吸、中心静脉压、四肢末梢循环的变化。
This paper summarizes the experience of perioperative nursing for 11 patients with thrombocytopenia undergoing open heart operation. Before operation,it is necessary to centralize the treatment and examination procedures and use intravenous indwelling needle to decrease bleeding due to puncture. After operation,nursing care focused on observation and treatment of bleeding and errhysis around the surgical wound and drainage tube,maintaining the drainage tube unblocked and close observation of the volume and color of drainage fluid within one to nine hours after operation,rapid infusion of blood thrombocyte to increase the number of blood thrombocyte and maintain normal coagulation mechanism,dynamic monitoring of vital signs and limb circulation to detect pericardial tamponade timely.
