摘要
目的探讨盆底康复训练对水中分娩产妇早期压力性尿失禁(SUI)的防治作用。方法按国际尿失禁咨询委员会提供的《尿失禁问卷表》,采用问卷调查方式,对水中分娩后42d和90d的初产妇进行调查和评估。将320例水中分娩产妇,配对分为阴道锥体训练组、凯格尔训练组、盆底生物电治疗组和对照组进行训练,每疗程6周。结果水中分娩产妇产后6周SUI发生率11.25%,较孕期41.88%明显降低(P<0.05);阴道锥体训练组、凯格尔训练组、盆底生物电治疗组康复训练后SUI发生率分别为3.80%、3.66%、1.33%,较产后训练前7.60%、13.42%、10.66%明显降低(均P<0.01);各训练组训练后SUI发生率均明显低于对照组17.85%(均P<0.05)。结论阴道锥体训练、凯格尔训练和盆底生物电治疗对水中分娩产妇早期SUI发生有一定的防治作用。
Objective To explore the prevention and treatment of water delivery early stress urinary incontinence by pelvic floor rehabilitation training. Methods By using the International Consultation on Incontinence Questionnaire, we investigated and assessed the primiparae at 42 days and 90 days after water delivery according to the standard. 320 primiparae were paired and divided into vaginal cone training group, Kegel training group, pelvic floor bioelectricity treatment group and control group. They were trained for six weeks every course, and the effects were assessed and evaluated before and after training. Results In each group, the incidence of early stress urinary incontinence 6 weeks after water delivery(11. 25 %)was decreased significantly as compared with that during pregnancy(41.88%, P〈0. 05). After training, the incidence of water delivery early stress urinary incontinence in vaginal cone training group, Kegel training group and pelvic floor bioelectricity treatment group was 3.80 %, 3.66%, 1.33% respectively, significantly lower than before training(7.60 %, 13.42 %, 10.66 % respectively, P〈0.01)and control group(17.85% ,P〈0.05). Conclusion Vaginal cone training,Kegel training and pelvic floor bioelectricity treatment have preventive and therapeutic effects on the genesis of water delivery early stress urinary incontinence.
