摘要
目的 探讨机体细胞免疫功能与乳腺癌发生、发展的关系及乳腺癌改良根治术对机体免疫功能的影响及免疫治疗在乳腺癌综合治疗中应用的意义。方法 采集 2 0名乳腺良性肿瘤患者 (术前 ) ,2 0名Ⅰ~Ⅱ期乳腺癌患者 (术前、术后 3天、术后 10天 )抗凝全血 ,应用流式细胞仪测定外周血中T淋巴细胞亚群CD3 + 、CD4 + 、CD8+ 细胞的百分比、CD4 + CD8+ 比率、NK细胞的百分比。结果 ①乳腺癌患者与乳腺良性肿瘤患者术前外周血中CD3 + 、CD4 + 、CD8+ 、NK细胞的百分比、CD4 + CD8+ 比率均在正常范围 ,两组比较无显著差异 (P >0 0 5 ) ;②乳腺癌伴有腋窝淋巴结转移组CD3 + 、CD4 + 及NK细胞百分比均略低于非转移组 ,CD8+ 细胞亚群百分比略高于非转移组 ,CD4 + CD8+ 比率亦略下降 ,但两组间无显著差异 (P >0 0 5 ) ;③术后 3天 ,CD3 + 、CD4 + 细胞百分比升高 ,CD8+ 降低 ,CD4 + CD8+ 升高 ,NK细胞略下降 ;术后 10天各项指标与术后 3天变化基本相同 ;术前与术后 3天、10天各组相比差异无显著性 (P >0 0 5 )。结论 早期乳腺癌患者机体免疫系统紊乱不明显 ,免疫功能没有明显下降。肿瘤对机体免疫抑制作用不显著。
Objective To investigate the relationship between organism immunologic function and generation and development of breast cancer,to study the effect of modified radical mastectomy for breast cancer on organism immunologic function.Methods Anticoagulative whole blood from 20 patients with benign breast tumour preoperatively and 20 patients with early breast cancer (Ⅰ or Ⅱ stage) preoperatively,3 days and 10 days after mastectomy was sampled.We detected CD3 +,CD4 +,CD8 +,CD4 +/CD8 + and NK cells from peripheral blood by using multicolor flow cytometry.Results ①There were no difference on the percentages of CD3 +,CD4 +,CD8 +,NK cells and the rates of CD4 +/CD8 + between early breast cancer patients and benign breast tumour patients ( P >0.05).②The percentages of CD3 +,CD4 +,NK cells and the rates of CD4 +/CD8 + in the patients with metastasis of axillary lymph nodes were mildly lower than those without metastasis of axillary lymph nodes,while the changes of CD8 + were just on the contrary,but there were no significant difference between the two groups ( P >0.05).③The percentages of CD3 +,CD4 + and the rates of CD4 +/CD8 + were slightly higher,and the percentages of CD8 +,NK cells were mildly lower in 3 days and 10 days postoperative groaps than in preoperative group.There was no obvious difference among three groups ( P >0.05).Conclusion Immunologic function of early breast cancer patients does not decrease significantly,and cancer does not suppress organism immunologic function obviously.Modified radical mastectomy for breast cancer has not marked effect on organism immunologic function.
