摘要
目的 :探讨骨盆骨折合并后尿道损伤急诊处理的效果。方法 :回顾性分析我院1981年 7月～ 1999年 9月收治骨盆骨折合并后尿道损伤的患者 30例 ,并对急诊处理的方法选择进行了探讨。结果 :后尿道挫伤者 10例 ,插入导尿管治疗 ,排尿正常 ;后尿道部分断裂者 1例 ,插导尿管失败后行膀胱造瘘术加尿道扩张术 ,排尿正常 ;后尿道完全断裂者 1例 ,急诊行尿道端端吻合术 ,排尿正常 ;后尿道完全断裂者 8例 ,行尿道会师加牵引术 ,7例排尿正常 ,1例重度尿道狭窄 (失败 ) ;后尿道完全断裂者 10例 ,因伤势重 ,病情不稳定 ,急诊单纯行膀胱造瘘术 ,3个月后除 2例失访外 ,余 8例均用不同的方法行二期手术治疗 ,术后效果满意。结论
Objective:To discuss the effectiveness of emergency treatment of posterior urethral injury caused by pelvic fracture.Methods:Retrospective analysis of 30 patients with posterior urethral injury caused by pelvic fracture from July 1981 to September 1999 was performed in order to explore the most suitable methods of emergency treatment.Results:Ten patients with simple contusion of posterior urethral rupture were treated by catheterized,and normal micturition was restored;1 patient of partial rupture of posterior urethral was treated by cystostomy and dilatiation of the urethra after a failure in catheterization, Normal micturition was also obtained.Eight patients with complete rupture of posterior urethral were treated by urethral realignment with traction,and 7 case resumed normal micturition except 1 case who developed urethral stricture failure;In other 10 cases with complete disrupture of urethra,only emergency cystostomy was performed because of severe and unstable general condition.Three months after the injury,a second operation procedure of different methods was performed in cases with satisfactory results.Two cases were lost to follow up.Conclusion:The prognosis of the injury of posterior urethral is closely related with the success of the emergency treatment.
