摘要
目的 评价理想牙关系指数(IdealToothRelationshipIndex-ITRI)的临床运用价值。方法 运用Haeger提出的ITRI评价方法,对44付安氏类和类错经标准方丝弓矫治前后的石膏模型进行ITRI的计测。结果 仅有后牙段ITRI和唇舌侧关系ITRI矫治前后的差异无统计学意义外,其余ITRI矫治前后的差异均有统计学意义。总ITRI从矫治前的34.7%上升到矫治后的54%。矫治后前牙ITRI的增加较后牙的增加为高。其中前牙上下接触的改善最大,从10.7%增加到78.6%,后牙段颊侧关系的改善高于舌侧关系的改善。结论 ITRI可对牙关系的状况进行量化分析,故为评价错畸形的严重程度和治疗效果的优劣提供了一种较为直观的量化参考指标,且可重复性较好。但ITRI分析也有其内在固有的不足。
Objectives To evaluate the value of Ideal Tooth Relationship Index (TIRI) in clinical applications. Methods Study models of 44 treated malocclusions were visually assessed to quantify the number of ideal tooth relationship actually present relative to the potential total. According to Haegers method, both intraarch and interarch relationships were used to generate an ideal tooth relationship index (ITRI). Results The difference between pre-and post treatment scores was statistically significant, except posterior, buccal and lingual scores. The total index scores improved from 34.7% to 54% as a result of orthodontic treatment. As a result of treatment the anterior segment showed higher index scores than the posterior. The interarch relationships of the anterior segment registered the highest (from 10.7% to 78.6%). Buccal relationships in the posterior segment produced higher scores than the lingual segment. Conclusion The situation of occlusion relationships can be quantitatively analyzed with ITRI, which offers a quantitative reference to evaluate the severity of malocclusion and the effectiveness of treatment results. The index scores were demonstrated to be reproducible and reliable. But there were intrinsic shortcomings in ITRI, so the attention must be paid to the application of ITRI.
