摘要
目的:观察益气清肝方联合拉米夫定治疗乙型肝炎肝硬化的疗效。方法:将58例患者随机分为2组,治疗组30例以益气清肝方联合拉米夫定治疗,对照组28例单用拉米夫定治疗,疗程52周。观察2组肝功能[谷丙转氨酶(ALT)、血清总胆红素(TBi)l、血清白蛋白(Alb)]、HBV DNA、Child-Pugh积分及乙肝两对半的变化。结果:治疗组患者HBV DNA阴转率高于对照组,2组比较,差异有显著性意义(P<0.05);2组HBeAg/HBeAb血清转换率、HBV DNA-YMDD变异率比较,差异无显著性意义(P>0.05)。2组患者治疗后ALT、TBil、Alb均较治疗前改善,差异均有显著性意义(P<0.05);治疗后治疗组ALT、TBil与对照组比较,差异均有显著性意义(P<0.05)。2组患者治疗后Child-Pugh积分均较治疗前改善,差异均有显著性意义(P<0.05)。结论:益气清肝方联合拉米夫定治疗乙型肝炎肝硬化疗效确切。
Objective= To observe the clinical effect of Yiqi Qinggan Decoction combined with lamivadine for chroni~ hepatitis B patients with liver cirrhosis. Methods. Fifty-eight patients were randomly divided into two groups. The treatment group with thirty cases was treated with Yiqi Qinggan Decoction combined with lamivadine, and the control group with twenty-eight cases was given lamivadine. The treatment lasted fifty-two weeks. The changes of liver function parameters including alanine aminotransferase（ALT）, total bilirubin（TBil） and albumin（AIb）, as well as hepatitis B virus（HBV）DNA quantitation, Child-Pugh scores and HBV markers were detected. Results= The negative conversion rate of HBV DNA in the treatment group was higher than that in the control group, the difference being significant（P 〈 0.05）.The differences of HBeAg/HBeAb seroconversion rate and HBV DNA-YMDD mutation rate were not significant between the two groups（P 〉 0.05）. After treatment, liver function （ALT, TBil and AIb）was improved significantly in both groups（P 〈 0.05 compared with that before treatment）. The improvement of ALT and TBil of the treatment group was more significant than that of the control group. After treatment, Child-Pugh scores in both groups were improved significantly（P 〈 0.05 compared with those before treatment）. Conclusion： Yiqi Qinggan Decoction combined with lamivadine has certain effect for hepatitis B patients with liver cirrhosis.
