摘要 目的探讨蝶骨嵴脑膜瘤的显微手术方法,提高手术疗效。方法回顾性分析2005~2010年间收治的32例蝶骨嵴脑膜瘤病例的临床资料,其中内侧型11例,外侧型21例。结果所有病例均接受显微外科手术,并按Simpson手术分级评定手术结果。肿瘤全切(Ⅰ、Ⅱ级)26例,Ⅲ级切除4例;Ⅳ级切除2例。无手术死亡,术后并发视力下降2例,不全瘫痪1例。结论选择准确的手术入路,采用精细的显微外科技术,在充分保全与肿瘤粘连血管、神经的前提下最大限度地切除肿瘤,可显著提高疗效,减少肿瘤复发。对未能全切病例,术后放疗可延缓肿瘤复发。 Objective This paper aims to investigate and discuss the use of microsurgical methods in treating sphenoid ridge meningioma in order to improve the effects of surgery. Methods This paper is going to conduct a retrospective analysis of the 32 cases of sphenoid ridge meningioma treated in our hospital between 2005 and 2010, of which 11 cases are medial and 21 are lateral. Results All the patients received microsurgery, and the results of the surgery are reviewed by Simpson operation grading system. Among them, there are 26 cases of total removal of tumors (level one and two), 4 cases of level three removals, 2 cases of level four removal. There is no operative death. 2 people were found to have hypopsia, and one incomplete paralysis. Conclusion Significant improvement of the treatment effects and reduction of tumor recurrence can be seen if the following things are ensured. 1) Selecting the correct surgical approach. 2) Using the refined microsurgical techniques. 3) Resecting tumors to a maximum extent under the condition of fully saving vessels that are adhered to tumors and the nerves. As to the incomplete tumor removal cases, postoperative radiotherapy can delay tumor recurrence.
