前交叉韧带残端引起伸膝受限的研究 被引量:2

Analysis of Extension Loss Caused by Remnants of Anterior Cruciate Ligament after Injury

摘要 目的:综合分析研究前交叉韧带损伤后残端引起伸膝受限的发生情况。方法:回顾性研究2008年6月至2010年4月536例入院治疗的前交叉韧带损伤患者,通过术中关节镜观察前交叉韧带残端增生卡压情况,同时观察患者伸膝受限情况及伴随的病史、体征、MRI及病理结果。结果:536例患者中有11例因前交叉韧带残端卡压引起伸膝受限。平均受限角度为9.1°(5°~15°),所有伴有伸膝受限的病变发现时间在伤后3周到6个月(平均6.8周)。术前MRI可以看到残端迂曲存在于髁间窝,术中观察断裂位置均位于上止点附近,残端形态为不规则团块,病理显示病变为排列紊乱的纤维增生结缔组织和韧带成分,未见软骨和骨样组织形成。结论:前交叉韧带损伤后残端卡压可引起伸膝受限,其形成与前交叉韧带断裂位置和损伤后残端翻转有关,发生率不高,应尽早治疗,术前MRI有助于诊断。 Objective The purpose of this study was to retrospectively investigate the characteris- tics of extension loss caused by remnants of anterior cruciate ligament (ACL) after injury. Methods Among 536 cases of ACL injuries from June 2008 to April 2010, 11 cases (male 8; female 3 ) were found exist extension loss caused by remnants of ACL, which were confirmed by arthroscopy. The time of extension loss occurrence, associated symptom and sign, MR/, arthroscopic findings were observed. Histological examination was done in the lesion resected. Results The average extension loss was 9.1° (range from 5° to 15°) . The average time from injury to operation was 6.8 weeks (range from 3 to 25 weeks) . In MRI examination, remnants of ACL depressed in the intercondylar notch was found. The rupture location of ACL were all close to upper insertion of ACL, agglomerated in shape. The microscopic examination of the resected remnants of ACL revealed disorganized fibrous connective tissue and composition of ACL ligament, but no chondroid and osseous tissues were found. Conclu- sion The remnants of ACL could cause extension loss, which could be diagnosed by MRI. The reason of formation was related to ACL rupture location and inverting of remnants. The incidence was low. Once it was diagnosed, operation should be done as soon as oossible.
