摘要
目的 探讨2型糖尿病患者血糖波动对心率变异性（HaV）的影响。方法以动态心电图中的LF、HF、SDNN、pNN50指标对56例2型糖尿病患者血糖波动24h的心率变异性进行对比分析，并选62名健康人作对照。结果2型糖尿病患者LF、HF、SDNN、pNN50指标在血糖波动前明显高于波动后，分别为（1324．6＋265．6）ms比（1196．5±208．7）ms，P=0．041，（764．6±123.3）ms比（698．2±112．5）ms，P=0．029，（161．1±19．3）ms比（118．2±10．4）ms，P=0．034，（39．3±6．2）比（31．2±5．9），P=0．046；但其血糖波动前各项HRV指标仍较健康人低，分别为（1621．8±275．1）ms比（1324．6±265．6）ms，P=0．028，（886．4±143．2）ms比（764．6±123．3）ms，P=0．025，（193．1±18．2）ms比（161．1±19．3）ms，P=0．038，45．4±7．0比39．3±6．2，P=-0．037。结论2型糖尿病患者血糖波动后HRV降低，表明血糖波动会造成心脏的植物神经功能受损。
Objective To analyze the relevance analysis of heart rate variability （HRV） and blood-sugar variance of the patients with type 2 diabetes mellitus （T2DM）. Methods 56 T2DM patients received dynamic electrocardiogram examination were chosen to research group, and 62 normal subjects examed by physical examination were chosen to control group. The indexs of LF, HF, SDNN, pNN50 were compared between two groups. Results Compared to pre-variance, the indexs of LF, HF, SDNN, pNN50 of post-variance were obviously higher,（1324.6±265.6）ms vs （1196,5±208.7）ms, P=0.041,（764.6±123.3）ms vs （698.2±112.5）ms, P=0.029, （161.1±19.3）ms vs （118.2±10.4）ms, P=0.034, （39.3±6.2） vs （31.2±5.9）, P=0.046, respectively. Compared to normal physical examination crowd, the indexs of LF, HF, SDNN, pNN50 of pre-variance were obviously higher, （ 1621.8±275.1 ）ms vs （ 1324.6±265.6）ms, P=0.028, （886.4±143.2）ms vs （764.6±123.3）ms, P=0.025, （193.1± 18.2）ms vs （161.1±19.3）ms, P=0.038,（45.4±7.0） vs （39.3±6.2）, P=0.037, respectively. Conclusion Bloodsugar variance could reduce HRV of the patients with T2DM and harm the cardiac autonomic nerve function.
