摘要 目的:探讨超声观察胎儿手结构的较好时期及方法。方法:对194例发育正常的孕13+0~17+6周胎儿共388只手进行超声检查,记录不同孕周胎儿手的掌骨、手指完全显示数及各指骨骨化数,并进行统计学分析。结果:胎儿手完整显示率在孕13+0~16+6周间均无显著性差异(P〉0.05);而在孕13+0~16+6周各孕周组与孕17+0~17+6周之间有显著性差异(P〈0.01)。孕13周开始所有掌骨、拇指2节指骨及其余4指近、远节指骨均骨化,而中节指骨随孕周增加按食指至小指顺序逐渐骨化,孕17周后所有指骨均骨化。结论:从孕13周起可对胎儿手进行初步观察,以提高严重畸形的检出率,孕17周后是超声观察指骨缺失或发育不良的较好时期。 Objective:To detect the better time and method for observing fetal hand structure with transabdominal ultrasonography.Methods:Three hundred and eighty-eight hands of 194 normal fetuses 13+0~17+6 weeks of gestation were observed by ultrasound.The number of metacarpales,fingers fully showed and phalanges ossification was evaluated and analyzed statistically.Results:There was no significant difference among groups of 13+0~16+6 weeks of gestation in complete display rate of hands(P0.05),but significant difference was found between 13+0~16+6 weeks and 17+0 ~17+6 weeks of gestation(P0.01).All metacarpales,phalanges of thumb,proximal phalanges and distal phalanges ossified from 13 weeks of gestation,and middle phalanges ossified gradually with gestational age.All phalanges ossified after 17 weeks of gestation.Conclusion:It is feasible to screen preliminary fetal hand for serious malformation from 13 weeks of gestation with ultrasound,the better opportunity for absence or hypoplasia of phalange observation is after 17 weeks of gestation.
