摘要
目的:探讨高颈段椎管内肿瘤手术治疗特点。方法:根据X线、CT及MRI等检查结果,初步评价肿瘤的性质、受累范围,确定手术方案。枕下颈后正中线入路15例,颈外侧入路3例,颈前方入路1例。结果:痊愈16例,1例术后无变化,1例术后呼吸困难加重,并发吸入性肺炎,术后3 d呼吸衰竭死亡。15例随访1～2年,肿瘤无复发。结论:合理的手术入路、熟练仔细的操作是手术成功的关键,显微手术是脊髓内肿瘤最有效的治疗方法。
Objective：To discuss the characteristics of surgical treatment of intraspinal tumor in the upper segment of cervical.Methods：The surgical planning of patient was made after the character,range of tumor were evaluated according to the results of X-ray,CT and MRI.Results：After the surgical treatment,16 patients recovered,one patient had no improvement,one patient got increasing dyspnea complicated by aspiration pneumonia and died from breath failure.There was no tumor recurrence case after 1 to 2 years of follow-up in 18 patients.Conclusions：Optimal surgical approach and skillful operation are key to the treatment of intraspinal tumor,and microsurgery is the most effective method for treatment of intraspinal tumor at present.
