双钢板经皮微创接骨板固定技术治疗复杂胫骨平台骨折 被引量:18

摘要 目的 探讨双钢板经皮微创接骨板固定技术(MIPPO)治疗复杂胫骨平台骨折的疗效. 方法 2006年1月至2009年1月共收治36例复杂胫骨平台骨折患者,男21例,女15例;年龄18 ~56岁,平均30.5岁.骨折按Schatzker分型:Ⅴ型20例,Ⅵ型16例,采用前外侧和内侧联合切口入路,应用MIPPO技术行内、外侧高尔夫棒型支撑接骨板内固定术.术后常规摄X线片评价骨折愈合及内固定物位置情况,记录膝关节活动度、切口愈合情况及美国特种外科医院膝关节评分等. 结果 36例患者术后获4 ~ 23个月(平均13.5个月)随访.患者伤口均一期愈合,骨折愈合时间为4~13个月,平均6.9个月,无再骨折发生.美国特种外科医院膝关节评分平均为86.8分(74 ~ 93分).4例(11.1%)患者出现成角畸形. 结论 双钢板MIPPO技术创伤小、并发症少、复位确实可靠、操作简单、骨愈合率高,是治疗复杂胫骨平台骨折较为理想的手术方法.同时,只有严格掌握手术操作原理,才能最大限度地降低并发症的发生率.
