摘要
目的探讨高血压患者尿微量清蛋白(MAU)与超敏C反应蛋白(hs-CRP)及其-717A>G基因多态性的关系。方法将高血压患者按尿清蛋白水平分为尿正常清蛋白(NMAU)组(75例)和MAU组(56例),另选择健康体检正常者为对照组(60例)。免疫浊度法测定各组血浆hs-CRP水平,聚合酶链反应-限制性片段长度多态性(PCR-RFLP)技术检测受检者CRP的-717A>G基因型及等位基因频率,并进行组间比较。结果 3组受检者的收缩压、舒张压、hs-CRP、MAU、总胆固醇(TC)、三酰甘油(TG)水平比较,差异均有统计学意义(P<0.05)。其中NMAU组与MAU组的收缩压、hs-CRP、MAU、TG水平比较,差异均有统计学意义(P<0.05)。Logis-tic回归分析显示hs-CRP升高是高血压患者MAU发生的危险因素(OR=3.107)。MAU组与NMAU组受检者比较,CRP-717A>G基因型AA、AG及GG频率分布差异无统计学意义(χ2=3.621,P>0.05);等位基因A、G频率分布差异有统计学意义(χ2=3.931,P=0.047)。结论 hs-CRP升高是高血压患者MAU发生的独立危险因素,CRP-717A等位基因可能是高血压患者MAU发生的易患基因。
Objective To investigate the association of microalbuminuria(MAU) with high-sensitivity C-reactive protein(hs-CRP) and its-717A>G gene polymorphism in hypertensive patients.Methods Hypertensive patients were divided into urine normal albumin(NMAU) group(75 cases) and MAU group(56 cases) by urinary albumin.60 healthy volunteers served as controls.The serum levels of hs-CRP of the subjects were measured,and the CRP-717A>G genotypes and alleles were screened by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP).All those results were compared between groups.Results The differences of systolic blood pressure,diastolic blood pressure,hs-CRP,MAU,TC and TG between the three groups were statistically significant(P<0.05).The concentrations of hs-CRP,MAU,triglyceride(TG) and systolic blood pressure in the MAU group was higher than those in NMAU group(P<0.05).Logistic analysis showed that higher hs-CRP was the risk factors of MAU in hypertension patients(OR=3.107).There were no significant differences in the frequency of CRP-717A>G genotype between the NMAU and MAU groups(χ2=3.621,P>0.05).And the frequency of CRP-717A allele in the MAU group was higher than that in NMAU group(χ2=3.931,P=0.047).Conclusion Higher hs-CRP was the independent risk factor of MAU in hypertension patients.CRP-717A allele may be a susceptibility gene of MAU in hypertensive patients.
