应力负荷下兔肩袖急性断裂重建术后腱-骨修复过程中生物力学特性的研究

Biomechanical properties of tendon-bone in rabbits with postreconstructive acute ruptured rotator cuff under stress

摘要 目的探讨应力刺激训练对兔肩袖急性断裂术后修复过程中腱-骨结构生物力学特性的影响。方法取新西兰白兔45只(2.0~2.5 kg),术前随机选取5只处死、行生物力学指标检测,作为对照。余白兔随机分为2组:A组(应力负荷组),B组(与非应力负荷组),A组术后第二周开始训练,B组正常笼养。分别于术后第2、4、6、8周最后1d训练结束后处死、取材,每组每次处死5只,并行生物力学指标检测。结果数据均采用x±s表示,数据采用SPSS 13.0统计软件处理。结果术后2、4、6周,组间比较,最大强度值(P=0.002、P=0.000和P=0.009)与最大载荷值(P=0.001、P=0.000和P=0.000)均存在差异,应力负荷组值较非应力负荷组大。刚度值在术后6、8周,应力负荷组与非应力负荷组之间均存在差异(P=0.000和P=0.000),应力负荷组刚度值比非应力负荷组大。结论一定的应力刺激可以改善兔肩袖急性断裂术后腱-骨修复过程中的力学性质。 Objective To explore biomechanical properties of tendon-bone in rabbits with postreconstructive acute ruptured rotator cuff deeply under stress.Methods 40 adult male New Zealand white rabbits(2.0 ~ 2.5 kg) were randomly selected to set up animal models of acute rotator cuff rupture,and then divided into 2 groups: stress group(group A,n=20) with the training from the 2nd week,and non-stress group(group B,n=20) with no any training.Another 5 rabbits were randomly selected as the control and taken the biomechanical assessment.Respectively the last day of 2,4,6,8 weeks after operation,5 animals in each group were taken and conducted the biomechanical assessment.The data were analyzed by SPSS 13.0 statistical software.Results The values of maximum intensity(P = 0.002,P = 0.000 and P =0.009)and maximum load(P = 0.001,P = 0.000 and P = 0.000) in stress group were higher than that in non-stress group respectively at 2,4,6 weeks after operation.The Stiffness values in stress group were higher than that in non-stress group respectively at 6,8 weeks after operation(P = 0.000 and P = 0.000). Conclusions The adequate stress stimulation can improve biomechanical properties of tendon-bone in rabbits with postoperative acute ruptured rotator cuff.
