肝癌组织中端粒酶活性半定量检测及其意义 被引量:6

Semiquantative detection of telomerase activity in primary hepatocellular carcinomas

摘要 目的 研究端粒酶活性的半定量检测方法及其作为原发性肝细胞癌 (hepatocellularcar cinoma ,HCC)肿瘤标志物的可能性。方法 采用以PCR技术为基础的TRAP(telomericrepeatamplica tionprotocal)方法 ,半定量检测了HCC及癌旁肝组织 40例 ,肝转移癌及癌旁肝组织 8例 ,肝良性肿瘤 8例和正常肝组织 7例中的端粒酶活性 ,活性强度分别定为“ +”、“ ++”和“ +++”。并将检测结果与肿瘤体积、数目、分化、有无包膜和门静脉癌栓、术后复发等临床病理指标进行对比分析。结果 本组HCC组织 40例中端粒酶表达阳性 37例 ,其阳性率为 93% ,其中活性强度为“ +”者仅有 3例 (3/37) ,端粒酶活性与肿瘤数目和门静脉癌栓相关 (P <0 0 0 1) ,与肿瘤体积、分化、有无包膜和术后复发率无关 ;本组HCC癌旁肝组织 40例中端粒酶表达阳性 11例 ,其阳性率为 2 8% ,其中活性强度为“ +”者有 9例 (9/ 11) ,伴门静脉癌栓、多结节、切缘小于 1cm和术后复发的HCC癌旁肝组织端粒酶表达阳性率显著高于无门静脉癌栓、单结节、切缘大于或等于 1cm和术后复发者 (P <0 0 5 ) ;肝转移癌 8例端粒酶活性全部阳性 ,其癌旁肝组织有 4/ 8端粒酶表达阳性。肝良性肿瘤 8例中仅有端粒酶表达阳性 1例 ;正常肝组织 7例端粒酶表达均阴性。结论 肝癌? Objective[WT5”BZ] To investigate the semiquantative detection of telomerase and its potential as tumor marker in human primary hepatocellular carcinomas (HCC).[WT5”HZ]Methods[WT5”BZ] A total of 111 human liver specimens,comprising 40 HCCs and the adjacent tissues,8 hepatic metastases and the adjacent tissues,8 benign liver lesions and 7 normal liver tissues,were examined for telomerase activity by semiquantative detection with TRAP assay based on PCR.[WT5”HZ]Results[WT5”BZ] 37 of 40(93%) HCCs were positive for telomerase activity which were rated +++ in 4,++ in 30 and + in 3,and 11 of 40(28%) adjacent tissuses specimens were positive for telomerase activity which were rated ++ in 4 and + in 7.Eight hepatic matestatic cancer and 4 adjacent tissues were positive.Telomerase activity were negative in 7 normal liver tissues and 7 of 8 benign liver lesions. Stronger telomerase activity was detected in the HCCs with multinodules and malignant thrombus( P <0.05), and there was significant correlation between the telomerase activity in peritumoral tissue and the higher recurrence rate,status of capsule, malignant thrombus and resecting margine of tumor ( P <0.05).[WT5”HZ]Conclusions[WT5”BZ] The results suggest that telomerase activity could be confirmed in a majority of HCC with seldom weakly detection, while weaker detection is found in a minority of peritumoral tissue, and telomerase activity may be a novel biomarker of prognosis and early diagnosis of HCC.
