先天性心脏病患儿氨基末端脑钠素原与肺动脉高压的关系 被引量:2

Relationship of serum N-terminal pro-brain natriuretic peptide and pulmonary artery pressure in pediatric patients with congenital heart disease

摘要 目的探讨氨基末端脑钠素原(NT-proBNP)与小儿先天性心脏病并肺动脉高压(脉高压)的关系。方法应用酶联免疫吸附测定法(ELISA)检测20例健康儿童(正常对照组)及46例先天性心脏病患儿血浆NT-proBNP浓度。用彩色多普勒超声心动图估测先天性心脏病患儿肺动脉压力,分为三组:无肺高压组18例、轻度肺高压组18例、中重度肺高压组10例。结果先天性心脏病无肺高压组NT-proBNP水平稍高于正常对照组,但差异无统计学意义(P>0.05)。先心病患儿血浆NT-proBNP随着肺动脉压升高而明显升高,差异有统计学意义(P<0.01)。结论先心病合并肺动脉高压患儿血浆NT-proBNP浓度升高,升高程度与肺动脉压呈正相关,血浆NT-proBNP检测对先天性心脏病合并肺高压具有特异的诊断价值,可作为先心病肺动脉高压评价的一个客观指标。 Objective To study the relationship of the serum level of N-terminal pro-brain natriuretic peptide (proBNrP) and pulmonary artery pressure in pediatric patients with congenital heart disease (CHD) complicated by pulmonary artery hypertension (PAH). Methods The serum levels of NT-proBNP were detected with enzyme-linked immuno- sorbent assay (ELISA) in 20 healthy children (the control group) and 46 children with CHD. The 46 children were estimated for pulmonary pressure by Doppler ultrasonography cardiogram and divided into three groups: group A (n=18, without PAH), group B (n=18, with mild PAIl), group C (n=10, with moderate to severe PAH). Results The levels of NT-proB- NP in group A were slightly higher than those in the control group, with no statistically significant difference (P〉0.05) The NT-proBNP level in children with CHD significantly increased as the pulmonary pressure increased (P〈0.01). Conclusion There is arising tendency of NT-proBNP level in children with CHD and PAH. Detection of serum NT-proBNP level has great value in the diagnosis of CHD complicated by PAH.
