摘要
目的评价颏下岛状瓣转移修复口腔颌面部软组织缺损的应用价值。方法 5例口腔颌面部恶性肿瘤,扩大切除原发灶后形成的软组织缺损以颏下岛状瓣转移修复,观察皮瓣存活情况及修复效果,同时探讨颏下岛状瓣应用相关问题。结果本组5例,其中4例未发生感染及坏死,1例因疤痕体质至皮瓣修复失败。所有男性患者未见胡须生长。5例患者供区外形良好,无仰头功能障碍、闭口功能障碍及下唇外翻。均随访6～12个月,4例未复发,1例局部复发,术后面神经功能均良好。结论颏下岛状瓣血供稳定且充足,组织弹性及颜色与口腔颌面部组织接近,皮瓣制取简便,手术时间短,成活率高,术后放疗不影响皮瓣成活,供区缺损可一期拉拢缝合,且外形及功能良好,对于老年患者尤其适用。
Objective To clinically evaluate the application value of submental island flap transfer in the repair of oral maxillofacial soft tissue defects.Methods Soft tissue defects formed after wide resection of primary tumors in 5 cases of oral maxillofacial cancer were repaired by submental island flap transfer.The vitality of flaps and the effects of repair were observed,and the related issues to the application of submental island flap were discussed.Results Among 5 cases,no infections and necrosis occurred in 4 cases,whereas flap transfer failed in 1 case due to scar constitution.Facial hairs were not found at the repair sites of all the male patients.All of these 5 cases resulted in good shape and appearance at the repair sites,without the problems including head up rotation dysfunctions,oral closure dysfunction or everted lower lips.All the cases were followed up from 6 months to one year,during which local recurrence was found in 1 case.The facial nerve was found function very well in all these cases.Conclusion The application of submental island flap has the following advantages：sufficient blood supply,close to the defects,the elasticity and color of the tissue are close to the adjacent oral maxillofacial tissue,easy to prepare,short surgery time,high vital rate of the flaps,no impaction to the flap vitality by postoperative radiotherapy,the defects can be sutured in one phase,and preserves good shape and function,which is particularly beneficial for elderly patients.
