摘要
目的利用CT灌注成像(CTPI)探讨原发性肝癌(HCC)的血流动力学改变及其特点。方法对26例HCC患者行CTPI检查,检测肝癌组织与正常肝组织肝血流量(BF)、肝动脉灌注量(HAP)、门静脉灌注量(PVP)、肝动脉灌注指数(HPI)、平均通过时间(MTT)、毛细血管表面通透性(PS)等参数,并进行统计学分析。结果与正常肝组织比较,肝癌组织BF、HAP、HPI、PS明显升高,差异均有统计学意义(P<0.01);PVP、MTT明显降低,差异均有统计学意义(P<0.01)。结论 CTPI可在显示肝脏及肝癌结构的同时,直观定量地反映肝癌病灶血流情况,对进一步明确诊断及制定治疗方案具有重要的参考价值。
Objective To study the hemodynamic characteristics of CT perfusion imaging(CTPI)in hepatic carcinoma (HCC).Methods 26 cases of HCC patients examine by CTPI and the cancer and normal liver tissue's blood flow(BF), hepatic artery perfusion(HAP),portal vein perfusion(PVP),hepatic artery perfusion index(HPI),mean transit time (MTT),capillary permeability surface(PS)were recorded.Results BF,HAP,HPI and PS of cancer tissue were signifi- cantly higher than that of normal liver tissue and the differences were statistically significant(P<0.01 ).PVP and MTT of cancer tissue were significantly lower than normal liver tissue and the differences were statistically significan(P<0.01). Conclusion CTPI can display anatomical structures in normal liver and cancer at the same time and directly quantitative reflect the blood flow response of cancer.CTPI has great value for further diagnosis and treatment.
