摘要
目的:进一步探讨食管床内胃代食管颈部吻合治疗食管癌的手术适用范围与改进措施。方法:对我院1996年至1998年采用食管床内胃代食管颈部吻合术治疗的61例食管癌患者进行研究,主要分析该术式的优缺点及手术操作的改进措施。结果:本组治愈出院59例。发生并发症4例(占6.6%),其中死亡2例(占3.3%),一例术中奇静脉损伤大出血死亡,另一例因术后严重肺部感染并吻合口瘘死亡;其余2例分别为肺不张与心律失常,经治疗痊愈。结论:该术式适用于胸部各段食管癌;通过改进术中操作,取得较好疗效。
Objective:To investigate the indication and improvement of the operative method for stomach replacing esophagus in esophageal bed with the cervical esophagogastrostomy to treat thoracic esophageal carcinomal.Methods:To study the advantages and improvement of this operation method by which 61 cases of thoracic esophageal carcinoma were treated,from 1996 to 1998 in our hospital.Results:59 cases were cured,and 4 cases had complication (the incidence is 6.6%).Two of them were dead (the mortality is 3.3%):one case died of haemorrhage due to injury of azygosvein in the operation and the other case died of serious infection of the lungs and anastomosis leakage after operation.The next two cases who had arrhythmia and pulmonary atelectasis were cured.Conclusion:This operation manner can be used to treat upper,middle and lower thoracic esophageal carcinoma,the curative effect is good.
