摘要
目的:探讨节段性分析法在胎儿心脏超声检查中的方法和临床价值。方法:用此法检查了236例具有先天性心脏病高危因素的胎儿,分析包括心房、心室、大血管的辨认及心房心室、心室大血管连接的判断。结果:17例超声检查有明显的心内结构异常,随访发现均为复杂性和严重性先心病,3例单纯性室间隔缺损漏诊。结论:①节段性分析法在胎儿心脏的超声检查中是必需的,它能帮助发现许多胎儿心脏的结构异常,这些多数是严重的、复杂性的先心病,对于妊娠筛选、分娩处理及产后干预有重要意义。②节段性分析法对单纯性先心病有局限性。③胎儿四腔心切面能显示心脏的许多解剖结构。
Objective:To evaluate the clinical method and value of sequential segmental analysis in fetal echocardiography.Methods:236 high risk fetuses for cardiac malformations were examined by sequential segmental analysis in fetal echocardiography.The analysis include recognition of the morphology and connections of the three segments of the fetus heart:atria,ventricles and great vessels.Result:17 fetuses were imaged structurally abnormal heart,that were all complex and serious congenital heart disease.3 fetuses with small VSD were missed.Conclusion:①Sequential segmental analysis is necessary in fetal echocardiography.It can detect a number of cardiac abnormalities prenatally,that are often complex and severe,and play an important role in preganancy screening,delivery management and postnatal interference.②Small cardiac abnormality may be omitted in this way.③The four-chamber view can provide detail anatomy,and is an important view of sequential segmental analysis.
