摘要
目的 探讨雷公藤多苷加辨证中药对小儿过敏性紫癜性肾炎凝血机制的影响。方法 纳入过敏性紫癜性肾炎患儿62例,分为中成药雷公藤多苷+辨证中药+香丹注射液组(中药组,32例)和肾上腺皮质激素+肝素+潘生丁组(西药组,30例),治疗12周,观察两组治疗前后D-二聚体(D-D)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、血小板(PLT)水平的变化。结果 中药组治疗后12周PLT、D-D、FIB水平低于治疗前(P<0.05),治疗12周后PT、D-D、治疗4周后FIB水平的下降幅度高于西药组(P<0.05)。结论 雷公藤多苷加辨证中药可影响小儿过敏性紫癜性肾炎凝血机制,其改善D-D、PT、FIB水平优于肾上腺皮质激素+肝素+潘生丁。
Objective To explore the effect of tripterygium glycosides and administration of Chinese medicine based on differentiation on the coagulation mechanism of Henoch-Schonlein purpura nephritis （HSPN） in children. Methods Sixty-two children cases of HSPN were randomized into two groups, Chinese medicine group of 32 cases was treated with tripterygium glycosides plus Chinese medicine based on differentiation and Xiangdan Injection, and western medicine group of 30 cases was treated with adrenocortical hormone plus heparin and persantine. The treatment was given 12 weeks of 30 cases. The level of D-dimer （D-D）, prothrombin time （PT）, activated partial thromboplastin time （APTT）, fibrinogen （FIB）, and the level of blood platelet （PLT） before and after treatment of both groups were observed. Results After treatment, the levels of PLT, D-D, PT, and FIB of the Chinese medicine group were lower than those of before treatment （P〈0.05）. The decrease of PT and D-D after 12 weeks of treatment, and the decrease of FIB after treatment for 4 weeks of the Chinese medicine group were more significant than those of the western medicine group, and the difference was statistically significant （P〈0.05）. Conclusion Tripterygium glycosides and administration of Chinese medicine based on differentiation have influence on the coagulation mechanism of HSPN in children, the improvement of D-D, PT, and FIB is superior to that of the adrenocortical hormone plus heparin and persantine group.
