摘要
目的研究比较舒芬太尼与芬太尼对妇科腹腔镜手术气腹时脑电双频指数(BIS)和血流动力学的影响,为临床药物选择提供参考。方法选择60例ASAI～II级要求行腹腔镜子宫切除的患者,随机分为舒芬太尼组(S)和芬太尼组(F),每组30例。两组均采用芬太尼+丙泊酚+罗库溴铵诱导一次顺利插管成功,S组在气腹前5分钟追加舒芬太尼10μg,F组追加等效剂量的芬太尼50μg,分别记录追加药物前,气腹镜置入时,气腹后5分钟,气腹后10分钟两组的收缩压(SBP)、舒张压(DBP)、平均动脉压(MAP)、心率(HR)和BIS。结果 F组在气腹后的5分钟和10分钟SBP、DBP、MAP、BIS与气腹前比较均显著升高(P<0.05),HR变化不明显。S组在气腹后5分钟和10分钟的SBP、DBP、MAP、HR、BIS均明显低于F组(P<0.05)。S组的SBP、DBP、MAP、HR、BIS与气腹前相比差异无显著性(P>0.05)。结论 BIS能有效地反映腹腔镜气腹时患者的麻醉深度,舒芬太尼对腹腔镜手术气腹时的循环稳定性明显优于芬太尼,更适合于腹腔镜手术。
Objective To study the sufentanil, compared with fentanyl of gynecological laparoscopic surgery when measured brain electrical experiments index and the dynamics of blood for clinical drug selection influence, to provide tb.e reference- Metlsods Select 60 cases ASA Ⅰ～Ⅱ requirements do Laparoscopic hysterectomy, the patients were randomly divided into groups of sufentanil, too （S） and fentanyl group （F）, each group of 30 patients. Each group all use fentanyl ＋ propofol＋rocuronium induced a smooth intubation success, in group S measured before 5min 10μg of sufentanil, group F equivalent of additional dose fentanyl 50μg, record before drug, respectively additional lens implantation, gasless laparoscopic when measured after 5min, after 10min two groups measured the systolic blood pressure（SBP）, diastolic pressure （DBP）, mean arterial pressure（MAP）, and heart ratc（HR） and brain electrical experiments index （BIS）. Results In group F the SBP,DBP, MAP, BIS after gasless laparoscopic are significantly increased （P〈 0. 05）, S group in 5 rain and after gasless laparoscopic 10 min, the SBP,DBP, MAP, BIS were significantly lower than F group （ P〈 0. 05）. The group S SBP,DBP, the MAP, HR, BIS mea ured before and no significant difference compared （ P 〉 0.05）. Conclusion BIS can effectively reflect laparoscopic） in patients with clinical anesthesia, the depth of sufentanil too, laparoscopic surgery on when gasless laparoscop- ic cycle stability obviously better than the fentanyl, the more suitable for laparoscopic surgery.
