摘要
目的:探讨隐睾并发精原细胞瘤的临床病理特征。方法:对2例手术切除的小肠精原细胞瘤进行临床、组织学及免疫组织化学观察。结果:肿瘤由大小基本一致的瘤细胞构成,被纤维间隔分隔成巢状或不规则条索状;细胞圆形或多角形,边界清楚;胞质较丰富,多数透亮,部分呈嗜酸性;核圆或椭圆,大而深染,基本位于中央,核分裂多见;可见灶状、片状或大片坏死;间质内纤维组织增生,可见淋巴细胞弥漫性或灶状浸润,部分肿瘤边缘可见残存附睾管。免疫组织化学示PLAP,CD117,HCG和Vim以及CD45RO均阳性,CK,EMA,CD20,AFP,Syn,C gA,HM B45及S-100均阴性。结论:精原细胞瘤的病理诊断应紧密结合临床资料进行。
Objective To investigate the clinicopathological characteristics cryptorchidism complicated with seminoma. Methods The clinical data,histopathology changes,and immunohistochemical findings of 2 cases of seminoma from cryptorchidism were ananalyzed. Results Microscopic examination showed uniform looking cells arranged in nests separated by delicate fibrovascular stroma.The tumor cells were round and large,which displayed abundant clear cytoplasm and sharply outlined cell membranes.Some lymphoid cells infiltrated around the tumor nests（the large majority of T-cell type）.Immunohistochemically the tumor cells displayed positive staining for PLAP,CD117,HCG,CD45RO,and Vim,negative for CK,EMA,CD20,AFP,Syn,CgA,HMB45,and S-100. Conclusion The pathologic diagnosis of seminoma should carefully take the clinical data into consideration.
