顽固性心力衰竭患者应用多巴胺与美托洛尔的疗效及预后 被引量:9

摘要 目的探讨顽固性心力衰竭患者应用多巴胺与美托洛尔联合治疗的临床疗效及预后。方法回顾性分析76例在我院住院NYHA心功能Ⅲ-Ⅳ级难治性心力衰竭患者。按治疗方案不同分为两组:对照组37例,给予常规抗心力衰竭药物治疗;研究组39例,在常规抗心力衰竭药物治疗基础上给予多巴胺与美托洛尔联合治疗。疗程为1个月,治疗前后两组患者分别进行6min步行试验和心脏超声检查,对比两组患者心功能改变。随访1年后,比较两组患者再入院及猝死发生率。结果与对照组相比,研究组在6min步行试验距离、心率、左室射血分数、左室舒张末期内径及每搏输出量上有明显改善(P<0.05);研究组再入院率、猝死率均低于对照组(P<0.05)。结论多巴胺与美托洛尔联合治疗难治性心力衰竭临床疗效明显,预后显著改善。 Objective To probe into the clinical curative effect of dopamine and metoprolol and prognosis in patients with intractable heart failure.Methods The patients with intractable heart failure(NYHA heart function III-IV grades,n=76) were retrospectively analyzed and they were divided into two groups according to the therapeutic plans:the control group(n=37) treated with routine anti-heart failure medications,and research group(n=39) treated with dopamine and metoprolol on the base of routine anti-heart failure medications.The treatment course was one month.The test of 6-minute walk and ultrasonic cardiogram were given to the patients in two groups before and after the treatment and heart function was compared.After follow-up for one year,the readmission rate and sudden death rate were compared between two groups.Results Compared with the control group,the distance of 6-minute walk,heart rate,LVEF,LVEDd and stroke volume were significantly improved in the research group(P0.05).The readmission rate and sudden death rate were lower in the research group than those in the control group(P0.05).Conclusion The integrative administration of dopamine and metoprolol has significant curative effect on intractable heart failure and prognosis is good.
