系统评价仰卧位与俯卧位在经皮肾镜取石术中的应用 被引量:8

摘要 目的系统评价仰卧位经皮肾镜取石术(PCNL)的安全性及有效性。方法搜集国外有关仰卧位与俯卧位经皮肾镜取石术与标准经皮肾镜取石术的随机对照试验文献,并追查已纳入文献的参考文献。对符合纳入标准的研究使用统计软件RevMan5.0完成Meta分析。结果经筛选,最后纳入5篇文献,受试患者共422例,进行Meta分析。与俯卧位PCNL组比较,仰卧位PCNL组患者:(1)平均手术时间缩短了24.76min(95%CI为-39.35~-10.17,P=0.000 9);(2)平均住院时间、术后输血率、发热、结石清除率及尿外渗方面比较差异无统计学意义(95%CI分别为-11.80~3.55,0.90~4.56,0.24~1.03,0.66~1.81,0.49~4.83)。结论仰卧位经皮肾镜取石术是一种安全可行的治疗上尿路结石的手术方法。本文因纳入文献和样本量有限,建议进行大样本、长期随访的高质量临床试验,为仰卧位PCNL的临床应用提供更佳的循证依据。 Objective To systematically review the safety and efficacy of supine percutaneous nephrolithotomy. Methods To retrieve controlled trials of supine and prone percutaneous nephrolithotomy,and investigate reference documents of literatures included. Data analyses of literatures which consistent with included standard were performed with the Cochrane ColLaboration's RevMan 5.0 software. Results Five literatures were retrieved,including 422 patients,done Meta-analysis. The Meta-analysis results showed that compared with control group,patients in test group: (1) decrease the mean operation time by 24.76 min(95%CI:-39.35- -10.17 ,P=0. 000 9) ;(2) there was no significant difference between the two ways with Mean hospital stay,post-operation transfusion, post operation fever, post-operation urine leakage, and stone free rate ( 95 % CI : -11.80 -- 3. 55,0. 90 -- 4. 56,0. 24 -- 1.03, 0.66-- 1.81,0.49-- 4.83). Conclusion Supine PCNL is a safe and effective method. Because of few studies and small samples,more high quality trials with large samples are proposed.
